For those of you who have the digital capability to collate VTE risk assessment compliance from all patients admitted aged 16 and over, do you include chemotherapy patients? I am having a lot of challenges with the inclusion of this patient cohort

Nice is quite clear and  states: 

Admission in the context of this guideline refers to admission as an inpatient, where a bed is provided for 1 or more nights, or admission as a day patient, where a bed is provided for a procedure including surgery or chemotherapy but not for an overnight stay.

We stipulate they should have a VTE risk assessment within 24 hours of admission, we don't use the 14 hours as we didn't think it feasible to achieve. This may change now NICE have introduced their tweaks  to the guidance which puts us under more pressure to achieve the 14 hour target and the 12 hour target for patients in ED. 

I think we are outliers still using the 24 target for VTE risk assessment.

they are in a cohort at NBT - admitted and discharged from same place and deemed low risk of VTE - in the same way we cohort dialysis patients.  not saying that they won't get a DVT , but that coming in for the chemo is low risk

Thank you everyone, we currently do not have a derogation policy in place for cohorts - does anyone have a policy for cohorts they could share with me please?

We do not have a policy for cohorts but follow the NHSE guidance - Ill try and find it again and sent to you

Basically - all patient who come to hospital seemed to be 'admitted' - even if not going to a bed and just having a dressing change for example, 

Our cohorts are in general - applicable to places -eg  daycase small plastics ops (MOHS), SDEC. ED (unless >12 hours)

Now that we have electronic prescribing as a forcing measure everywhere, except those areas mentioned (and perhaps some other areas that can be identified on CMM- electronic prexcribing platform) practically all patients need a VTE RA before any drug can be prescribed - including anaesthetic drugs

I hope that that helps - having cohorts has really help with VTE RA compliance

As NICE NG89 includes day case and chemo as definition of an admission, I ideally want it covered for external reporting  and data collection. We dont currently cohort
