Diagnosis is a critical cornerstone of patient safety, impacting treatment decisions and health outcomes across the globe. With diagnostic errors contributing to nearly 16% of preventable harm in health systems, the need for accurate and timely diagnoses has never been more urgent. Inline with World Patient Safety Day 2024’s theme, “Improving Diagnosis for Patient Safety," and under the slogan “Get it right, make it safe!," we are focusing on transforming diagnostic processes to enhance patient safety and care quality.
WPA is excited to announce the WPSD webinar on Diagnosis scheduled on September 18 at 9 AM ET. This event will bring together leading experts, patients and patient advocates to explore innovative strategies, share best practices, and discuss how to foster collaboration among healthcare providers, patients, and families to minimize diagnostic errors.

The predominant approach to improving safety in healthcare has involved studying adverse outcomes to identify system vulnerabilities and correct them. While this approach is useful, it has limitations. As a result of continuously investigating and analysing errors, patient safety professionals can experience stress and may view their systems and teams as unusually unsafe.
This webinar will articulate the value of Success Cause Analysis (SCA) in promoting learning and organisational culture change. They will describe SCA considerations for each component of traditional event analysis, how they established SCA within their organizations, and how you can deploy SCA in your facility. Additionally, participants will learn how to mitigate “third victim syndrome” for safety professionals analysing only errors and how to involve patients and families in patient safety and quality process reviews.

As doctors, the GMC tells us that we must speak up if we observe patient safety concerns.
But too many doctors who make protected disclosures become victimised by bad employers. The series of actions deployed in these cases is the Employers' Playbook.
This webinar aims to equip you with the knowledge you need to protect yourself when making a protected disclosure. It will cover:
The clinical 'mafia' and how it works. How to spot the signs that you're being investigated. MHPS investigation and pre-investigation. How to safely raise a concern. What support is available. Register
