This forum will explore the barriers to successful implementation of the Learning from Deaths Guidance and what you can do within your organisation to help overcome these in order to successfully implement change and improve patient safety.
In addition, the role of Medical Examiners will be looked at, and, if published, a discussion on the introductory version of the Patient Safety Incident Response Framework (PSIRF) which will eventually replace the Serious Incident Framework, as outlined in the NHS Patient Safety Strategy, in 2021.
The session will have a practical focus with an opportunity for delegates to share good practice.
