    A trade union has written to every politician representing the Scottish Borders to highlight "dangerous staffing levels" in local hospitals. Unison claims serious breaches of safety guidelines are occurring daily due to a lack of nurses, auxiliaries and porters. The letter says staff are unable to take proper rest breaks or log serious incidents in the reporting system. NHS Borders said patient and staff safety was its number one priority. Unison said working conditions in the area were regularly in breach of regulations. Greig Kelbie, the union's regional officer in the Borders, said: "We are getting regular messages from our members to tell us about the pressure they are under - and that they can't cope. "The care system was under pressure before Covid, but the pandemic has exasperated the situation, particularly at NHS Borders. "The NHS has been stretched to its limits and it is now at the stage where it is dangerous for patients and staff - we're often told about serious breaches of health and safety, particularly at Borders General Hospital where there are issues with flooring and staff falling. "We work collaboratively with NHS Borders to do what we can, but we also wanted to make politicians aware of how bad things have become. "We need our politicians to step up and implement change - we want them to make sure the Health and Care Act is brought to the fore and that it protects our members."     A ‘significant flurry’ of senior doctors will look to retire after a trust rejected a scheme designed to avoid higher taxes on their pensions, according to internal emails. Emails from senior doctors suggest Liverpool University Hospitals Foundation Trust has rejected proposals to introduce “pension recycling”, a scheme endorsed nationally to avoid the tax rules. Government policy changes in 2016 reduced tax relief on higher earners’ pension contributions. This has discouraged clinical consultants from taking on extra shifts, and in some cases, prompted people to retire earlier than planned. Last month, NHS England’s chief executive Amanda Pritchard suggested this was a continuing problem for hospitals as they seek to increase elective activity.     The Health and Social Care Committee examines the Government’s progress against its pledges on the health and social care workforce and will be the focus of a new independent evaluation by the Health and Social Care Committee’s Expert Panel. Professor Dame Jane Dacre, Chair of the Expert Panel, said: “We’ll be looking at commitments the Government has made on workforce – the people who deliver the health and social care services we rely on. “We’ve identified a recurrent theme in our evaluations to date – whether in maternity, cancer or mental health services, progress is dependent on having the right number of skilled staff in the right place at the right time. Shortages have a real impact on the delivery of services and undermine achievements. “Our panel of experts will evaluate progress made to meet policy pledges in this crucial area - whether it’s about getting workforce planning right, training, or ensuring staff well-being.” The Expert Panel will focus on three areas: Planning for the workforce – including how targets are set, recruitment, and retention. Building a skilled workforce – including incorporating technology and professional development of staff. Wellbeing at work – including support services for staff, and reducing bullying rates. Four specialists have been appointed for this evaluation, bringing their subject specific expertise and experience. They will work alongside the core members of the Expert Panel in identifying a set of Government commitments on workforce and evaluate progress made against them. The findings will support the work of the Health and Social Care Committee which is carrying out a separate inquiry: Workforce: recruitment, training and retention in health and social care.     Despite workforce being the biggest challenge facing the health service, the Health and Care Bill provides no clarity on the numbers of staff this country needs, says Andrew Goddard in a HSJ article. The Health and Care Bill returned to the Commons this week – as did the question of workforce planning. At the end of the spring term, MPs voted to reject an amendment to the bill which would have required the secretary of state to publish independent assessments of current and future workforce numbers every few years. The following week, the House of Lords – led by Baroness Cumberlege, with support from Baroness Harding, Lord Stevens of Birmingham and other cross-party peers – voted to put a revised version of the amendment back in. This particular game of ping pong about how we should plan the NHS and social care workforce is an important one. Workforce is not only a blindspot in the bill – it is a blindspot in the government’s ambitions for health and care. A lack of staff risks undermining the true potential of the Health and Social Care Levy because there will be too few staff to carry out the additional checks and diagnostic procedures promised. The new diagnostic hubs are to be staffed with existing NHS colleagues. Workforce shortages hampered our response to the pandemic and are already having a significant impact on our response to the backlog. They were also identified in the Ockenden Report as a driving factor in the avoidable deaths of 201 babies. It is concerning then, that despite workforce being the biggest challenge facing the health service, the Health and Care Bill provides no clarity on the numbers of staff this country needs.     The number of GPs in England has fallen every year since the government first pledged to increase the family doctor workforce by 5,000, a minister has admitted. There were 29,364 full-time-equivalent GPs in post in September 2015, when the then health secretary, Jeremy Hunt, first promised to increase the total by 5,000 by 2020. However, by September 2020 the number of family doctors had dropped to 27,939, a fall of 1,425, the health minister Maria Caulfield disclosed in a parliamentary answer. And it has fallen even further since then, to 27,920, she confirmed, citing NHS workforce data. In the 2019 general election campaign, Boris Johnson replaced Hunt’s pledge with a new commitment to increase the number of GPs in England by 6,000 by 2024. However, Sajid Javid, the health secretary, admitted last November that this pledge was unlikely to be met because so many family doctors were retiring early. Organisations representing GPs say their heavy workloads, rising expectations among patients, excess bureaucracy, a lack of other health professionals working alongside them in surgeries, and concern that overwork may lead to them making mistakes are prompting experienced family doctors to quit in order to improve their mental health and work-life balance. The British Medical Association (BMA) said the figures Caulfield cited showed that the lack of doctors in general practice was “going from bad to worse for both GPs and patients”, and it warned that patients were paying the price in the form of long waits for an appointment. “Despite repeated pledges from government to boost the workforce by thousands, it’s going completely the wrong way,” said Dr Kieran Sharrock, the deputy chair of the BMA’s GP committee. “As numbers fall, remaining GPs are forced to stretch themselves even more thinly, and this of course impacts access for patients and the safety of care provided.” Read full story Source: The Guardian, 11 April 2022

    A shortage of more than 2,000 midwives means women and babies will remain at risk of unsafe care in the NHS despite an inquiry into the biggest maternity scandal in its history, health leaders have warned. A landmark review of Shrewsbury and Telford hospital NHS trust, led by the maternity expert Donna Ockenden, will publish its final findings on Wednesday with significant implications for maternity care across the UK. The inquiry, which has examined more than 1,800 cases over two decades, is expected to conclude that hundreds of babies died or were seriously disabled because of mistakes at the NHS trust, and call for changes. But NHS and midwifery officials said they fear a growing shortage of NHS maternity staff means trusts may be unable to meet new standards set out in the report. “I am deeply worried when senior staff are saying they cannot meet the recommendations in the Ockenden review which are vital to ensuring women and babies get the safest possible maternity care,” said Gill Walton, chief executive of the Royal College of Midwives (RCM). The number of midwives has fallen to 26,901, according to NHS England figures published last month, from 27,272 a year ago. The RCM says the fall in numbers adds to an existing shortage of more than 2,000 staff. Experts said the shortage was caused by the NHS struggling to attract new midwives while losing existing staff, who felt overworked and fed up at being spread too thinly across maternity wards.     The chief nursing officer for England has spoken about the ongoing shortages of nurses across the country and how the government’s previous pledge for 50,000 more nurses is now “not enough”. At one of her first in-person speeches since the start of the pandemic, Ruth May also revealed that she thought the removal of the student nurse bursary in England was “fundamentally the wrong decision”. Ms May was speaking at an event organised by the League of St Bartholomew’s Nurses in honour of pioneering nurse Pam Hibbs, who died following a Covid-19 infection last year. During her keynote address, Ms May said workforce remained a “big focus” for her team due to the “shortage” of nursing staff nationally. She said work to address the gaps was centred on the three areas of international recruitment, domestic training, and retention. She said numbers of nurses being recruited from overseas annually had risen from around 5,000 to 6,000 before the pandemic, to an expected 20,000 in this financial year. “I’m very, very glad that the NHS has had a diverse workforce from its very inception.     The former health secretary Jeremy Hunt will join doctors’ representatives today in a call to stem the “bleed” of GPs or risk endangering patients. Polling shows that almost nine in ten GPs fear that patient safety is being put at risk by shortages of family doctors and too little time for appointments. The government has admitted that it will fail to fulfil an election pledge to recruit 6,000 extra full-time GPs by 2024. Hunt is campaigning with the British Medical Association and the GPDF, which represents local medical committees, in calling for the government to put forward a GP workforce plan to “rebuild general practice”. He said: “The workforce crisis is the biggest issue facing the NHS. We can forget fixing the backlog unless we urgently come up with a plan to train enough doctors for the future and, crucially, retain the ones we’ve got. “As someone who tried hard to get more GPs into local surgeries but ultimately didn’t succeed because the numbers retiring early exceeded those joining, I’m passionate about fixing this.” The campaign wants the government to deliver on its pledge for an extra 6,000 GPs in England and action to tackle the reasons for GPs leaving the profession, such as burnout. It says that a plan is needed to reduce GP workload, which would improve patient safety.

    A trust has admitted it is having to discharge patients inappropriately into care homes or community hospital beds because of a shortage of home care workers. A report to East Kent Hospitals University Foundation Trust’s board last week revealed that 160 extra beds had been commissioned to maintain flow across the local health economy “due to insufficient domiciliary/care package capacity.” It went on: “The clinical commissioning group have tried via Kent County Council to commission additional domiciliary care without success. It is acknowledged by the local health economy that it is important to withdraw from these additional beds as quickly as possible as they are not a cost-effective resource and more importantly, in many cases, they are not the ideal discharge destination for those patients who could have been discharged home with a care package. “Patients are being transferred into community hospital beds or residential home beds due to a lack of domiciliary care packages. Although this is a national issue, it will not be resolved locally until appropriate pathway capacity is commissioned.” Professor Adam Gordon, president elect of the British Geriatrics Society, said: “If people have been sent to a care home when they don’t want or need to be there that can affect their motivation and result in a form of deconditioning. The committee of MPs, which monitors spending across Whitehall, acknowledges Covid-19’s role in contributing to the ballooning backlog and lengthening waiting times. But it singled out years of inaction by the Department of Health and Social Care (DHSC) for particular blame.     The highest ever number of medical students have been told there are no places for them this year, despite the health service’s crippling shortage of medics. The risk that young would-be doctors may not be allocated to start their training at a hospital in the UK has sparked concern among the medical students affected, as well as medical organisations. Pressure is growing for action to close the gap between the number of training places available across the NHS and the number of graduates seeking one, so medical talent is not wasted and hospitals hire as many fresh recruits as they can to help tackle the widespread lack of medics. Doctors are worried that the mismatch between demand for and supply of training places will lead to the NHS missing out on medics it sorely needs and that some of those denied a place will either go to work abroad instead or give up medicine altogether. The most recent official figures showed that the NHS in England is short of almost 8,200 doctors. Dr Dustyn Saint, a GP in Norfolk, tweeted the health secretary, Sajid Javid, about the situation, saying: “Sajid Javid sort this out! You know how much general practice needs these people in a few years, standing by and doing nothing is inexcusable.” Another doctor said: “It’s bonkers that 800 would-be doctors could be denied training places at a time when the NHS in England is short of 8,200 doctors.” The British Medical Association has voiced concern about the large number of unallocated medics. “Now we have a situation where a record number are left with unnecessary uncertainty about where they are headed this August,” said Khadija Meghrawi, the co-chair of its medical students committee. “In a time where student mental health is declining, this additional source of uncertainty and stress is particularly unfair.” Read full story Source: The Guardian, 15 March 2022

    The number of referrals for specialist NHS mental health care reached a record high in England by the end of 2021, an analysis suggests. The Royal College of Psychiatrists says the pandemic has led to unprecedented demand and backlogs and services are struggling to keep up. There were 4.3 million referrals, for conditions such as anxiety and depression, in 2021, NHS Digital says. Just under a quarter - 1.025 million - were for children or adolescents. The college said the NHS had delivered 1.8 million mental health consultations in December 2021, but an estimated 1.4 million people were still waiting for treatment. And hundreds of adults were being sent far from home for treatment because of a lack of beds in their area. President Dr Adrian James said: "As the pressure on services continues to ratchet up, the silence from government continues to be of grave concern for the college, the wider mental health workforce and, most importantly, our patients. "The warning of the long tail of mental ill health caused by the pandemic has not been heeded. "Many thousands of people will be left waiting far too long for the treatment they need unless the government wakes up to the crisis that is engulfing the country. "Staff are working flat out to give their patients the support they need but the lack of resources and lack of staff mean it's becoming an impossible situation to manage... "...We need a fully funded plan for mental-health services, backed by a long-term workforce plan, as the country comes to terms with the biggest hit to its mental health in generations."     The NHS is facing a deepening staffing crisis, with the number of unfilled posts across health services in England rising to 110,192, official figures show. The shortages include 39,652 nurses and 8,158 doctors, according to the latest quarterly data for health service vacancies published by NHS Digital. The disclosure prompted warnings that the shortage of frontline personnel would lead to longer delays, hit the campaign to cut the 6.1m treatment backlog and undermine quality of care. Staff groups said they feared that low pay, burnout from heavy workloads and constant pressure during shifts, compounded by the Covid-19 pandemic, were leading demoralised workers to quit. “The fact that nursing vacancies remain stubbornly high, at about 40,000 in the NHS in England, is deeply worrying. With every job that remains unfilled, safe patient care becomes even harder to maintain”, said Patricia Marquis, the Royal College of Nursing’s director for England. Wes Streeting, the shadow health secretary, said: “The Conservatives’ scrapping of the nursing bursary and failure to fix staffing shortages has been disastrous for the NHS, and patients are paying the price. NHS staff do heroic work but there simply aren’t enough of them.     The NHS is “flying blind” and “woefully unprepared” to cope with England’s rapidly ageing population, senior doctors have warned as stark new figures reveal the country has only one full-time geriatrician to care for every 8,000 older people. The Royal College of Physicians (RCP) said the drastic shortage of specially trained physicians to look after the rising number of elderly people and a lack of NHS workforce planning meant England was “sleepwalking into an avoidable crisis of care for older people”. Its analysis of NHS and Office for National Statistics data shows there is just one full-time geriatrician for every 8,031 people over the age of 65 in England. There are also regional disparities, with one geriatrician caring for more than 12,500 over-65s in the east Midlands, while the figure in north-east and central London is one per 3,254. Estimates suggest that by 2040 there could be as many as 17 million over-65s. But the college warns that many doctors will soon be requiring geriatric care themselves as 48% of consultant geriatricians are due to retire within the next decade. The RCP said the health service was short of staff across all specialities and the shortage of geriatricians was one example of why the health service needed more workforce planning. It said there was no publicly available data on the number of staff the NHS needed to train now to meet future demand for care. Dr Jennifer Burns, the president of the British Geriatrics Society, said the crisis would only worsen with the “predictable rise” in the numbers of older people across the country. “It is absolutely vital that these fundamental issues around the recruitment, retention, development and support of the workforce are addressed, and that there is a properly resourced strategy for future needs,” she said.     The White House has announced plans to boost nursing home staffing and oversight, blaming some of the 200,000-plus covid deaths of nursing home residents and staff during the pandemic on inadequate conditions. Officials said the plan would set minimum staffing levels, reduce the use of shared rooms and crack down on the poorest-performing nursing homes to reduce the risk of residents contracting infectious diseases. The White House also said it planned to scrutinise the role of private equity firms, citing data that their ownership was linked with worse outcomes and higher costs. Nursing homes have been an epicenter of covid spread during the pandemic, as the virus initially tore through facilities before vaccines were available in 2020, and then continued to sicken and kill residents at an elevated rate last year. Advocates have demanded better policies to ensure the facilities are prepared for emergencies and follow practices to curb the spread of infections. Under Biden’s plan, officials at the Centers for Medicare and Medicaid Services will propose minimum staffing levels within the next year, which the White House said would improve safety by ensuring residents receive sufficient care and attention. The administration also cited a study that found increased staffing levels were linked with fewer covid cases and deaths. The nursing home industry has warned that the pandemic has exacerbated long-running staffing shortages, noting that roughly 420,000 employees in nursing homes and long-term care facilities, many of whom complained about low pay, have departed over the last two years.     A record number of more than 400 workers in England have left the NHS every week to restore their work-life balance over the last year, according to a new analysis of the workforce crisis hitting the health service. The flood of departures comes with staff complaining of burnout and cases of post-traumatic stress disorder following two years of battling the Covid pandemic. There are now concerns that the exodus is impacting the quality of care, with more than a quarter of adults saying they or an immediate family member had received poor care as a result of the workforce problems. The findings emerged in an assessment of the health service compiled by John Hall, a former strategy director at the Department of Health and Social Care, for the Engage Britain charity. Concerns over the state of the workforce came top of its list as it investigated the public’s attitude towards health and social care services, which remain under pressure in the wake of the pandemic. “The workforce crisis in the NHS has clearly penetrated the public consciousness,” Hall writes. “The UK has long had significantly lower numbers of doctors and nurses per capita than comparable systems … More recently, the impact of working conditions is showing an increasing impact on the ability of the NHS to retain staff. Around 50 in every 10,000 staff working in hospital and community health services in June 2021 left the service within the next three months, citing work-life balance as the reason. This was a new record.” Julian McCrae, Engage Britain’s director, said frontline health and care workers were now “running on empty” and a plan for boosting the workforce was overdue. “NHS workers across the country have spoken to us about feeling overstretched, undervalued and struggling to get support in a chaotic system,” he said. “We can’t allow staff to burn out, while putting patients at risk of mistakes or spiralling downwards as they wait months for treatment. The government must act quickly to expand its promise of reform, based on listening to the people who use or work in the system every day.     The NHS is facing a “time bomb” and will be forced to cancel or delay around 8 million operations each year by 2040, due to a lack of consultant anaesthetists across the services. The Royal College of Anaesthetists (RCOA) said the current shortage of at least 1,400 staff across the UK means millions operations will not be able to take place. The college has warned its speciality is facing a “perfect storm” of limited training places, poor retention and an ageing workforce with 39 per cent nearing retirement age. The analysis found as demand for surgeries continue the need for anaesthetists is due to increase by 3.85 per year, meaning the NHS will need around 25,000 doctors in these posts by 2040. Dr Fiona Donald, president of the RCOA said: “The NHS is facing an anaesthetic workforce time bomb. We already have profound workforce shortages that are preventing huge numbers of operations from taking place – and unless urgent action is taken, the problem is going to worsen. “We would welcome government funding for additional anaesthetic training posts. One hundred additional posts per year would start to plug the gap and help get the UK back on a sound footing to be able to address the waiting list backlog. Without this investment, we foresee impacts to patient care and a further impact on the mental health of our current workforce – they need to be able to prioritise their own health and that of their families alongside the focus they already place on the health of patients and the public.” Read full story Source: The Independent, 22 February 2022

    The government has rejected calls for an overhaul of NHS workforce planning amid concerns of staff shortages and a mounting backlog of patients. It comes after a House of Commons health and social care committee report in 2021 found burnout among nurses and other healthcare professionals had reached an emergency point. MPs had called for immediate action to support exhausted staff through a plan to cover staffing needs for the next two decades, led by Health Education England. But in a government response to the report, the Department of Health and Social Care (DHSC) rejected calls for independent annual reports on workforce shortages and future staffing requirements. Instead, a new duty in the recently introduced Health and Care Bill will require the health and social care secretary to publish reports on workforce planning in England every five years. The duty is intended to compliment ‘investment on workforce planning and supply already underway’, the government’s response states. But UNISON national nursing officer Stuart Tuckwood said a lack of an independent view on what is needed to support the NHS workforce risked the government focusing on cost ‘above all else’.     Amanda Pritchard has said it is time to ‘look again’ at whether NHS England should be given formal powers to disbar managers for ‘serious misconduct’. In an email to regional leaders and some national bodies yesterday, seen by HSJ, the chief executive officer of NHS England said the murder trial of neonatal nurse Lucy Letby has brought the issue of professional regulation for managers back into focus. She has planned an urgent meeting next week to discuss the options. Ms Pritchard said she wanted the meeting to explore; the feasibility of NHSE being given the powers and resources to act as a regulator; who this could apply to and how it could operate; and how a dedicated regulatory body for NHS leaders might fulfil the role. She stressed any new powers would need to be determined by the government, but said the NHS “should contribute proactively and fully, and with an open mind, to this decision-making process”.     The government has announced £250m in funding to provide an extra 5,000 NHS hospital beds in England this winter. Ministers say 900 new beds should be ready by January, with the remainder to follow soon after, boosting capacity and helping lower record waiting lists. The increase will mean nearly 100,000 permanent beds on wards and in A&E, available at the busiest time of the year - a 5% rise on current levels. NHS Providers said the extra capacity was needed "before winter begins". Miriam Deakin, director of policy and strategy at NHS Providers, said trusts would welcome the support but cautioned any new beds would need to be staffed. She added that, since winter is the busiest time of the year for urgent and emergency care, trust leaders would be concerned that the promised extra capacity is only expected to be in place by January. "For the best results, trusts would need these new beds before winter begins," she said. Pat Cullen, from the Royal College of Nursing, added: "The elephant in the room is who will staff these additional beds? Nursing staff are already spread too thinly over too many patients."     Racism is “a stain on the NHS” and tackling it is key to recruiting and retaining staff, the outgoing president of the Royal College of Psychiatrists (RCP) will warn. The health service has a moral, ethical and legal duty to do more to stamp out racism, Dr Adrian James is expected to say at the college’s international congress in Liverpool. He will cite pay gaps, disparities in disciplinary processes and a “glass ceiling” for doctors from minority ethnic backgrounds who want to progress into management positions as problems in the NHS that are linked to racism. Last month, the NHS Race and Health Observatory, which was formed in 2021 to examine disparities in health and social care based on race, said better anti-racism policies could strengthen the NHS workforce. The RCP agreed that “better care, training and anti-racist policies” would increase staff numbers in the NHS, and that this would “improve patient experience and save millions of pounds spent annually on addressing racism claims brought by staff, clinicians and patients”.     Nearly half of all NHS hospital maternity services covered so far by a national inspection programme have been rated as substandard, the Observer can reveal. The Care Quality Commission (CQC), which regulates health and care providers in England, began its maternity inspection programme last August after the Ockenden review into the Shropshire maternity scandal, which saw 300 babies left dead or brain damaged by inadequate NHS care. Of the services inspected under the programme, which focuses on safety and leadership, about two-thirds have been found to have insufficient staffing, including some services that were rated as good overall. Eleven services saw their rating fall from their previous inspection. Dr Suzanne Tyler of the Royal College of Midwives said: “Report after report has made a direct connection between staffing levels and safety, yet the midwife shortage is worsening. Midwives are desperately trying to plug the gaps – in England alone we estimate that midwives work around 100,000 extra unpaid hours a week to keep maternity services safe.     Healthcare leaders have called for an urgent plan to tackle the social care crisis, warning Rishi Sunak there is “clear concern” over an ongoing failure to tackle staff shortages. The warning from Matthew Taylor, chief executive of NHS Confederation which represents hospitals and community services, comes after the publication of the long-awaited £2.4bn NHS workforce plan, which committed to 300,000 extra nurses and doctors in the coming years. Mr Taylor said any benefits to improve NHS staffing will be “limited” without an equivalent strategy for the social care sector, which currently has 165,000 vacant posts. Health bosses, represented by NHS Confederation, have now written to the prime minister asking for “urgent intervention” and calling for a clear plan for improving pay and conditions to attract staff. Martin Green, chief executive for Care England that represents care homes, warned that the sector “is in the midst of a workforce crisis, which is going to get worse not getting better”. He welcomed the NHS Confederation’s letter and said unless similar improvements were made within social care, there would be more “cancelled operations, more people languishing in hospital when they don’t need to and the whole breakdown of the system”.     The head of NHS England has warned that July's planned strikes in the health service could be the worst yet for patients. Amanda Pritchard said industrial action across the NHS had already caused "significant" disruption - and that patients were paying the price. This month's consultant strike will bring a "different level of challenge" than previous strikes, she said. Junior doctors and consultants will strike for a combined seven days. Ms Pritchard told the BBC's Sunday with Laura Kuenssberg programme that the work of consultants - who are striking for the first time in a decade - cannot be covered "in the same way" as junior doctors.
