    The Inaugural Australian Patients for Patient Safety Workshop, held over 3 days in Perth from July 7 ‐9, 2009, brought together a group of 40 health consumers, many of whom had experienced medical error or health system failure, health providers and health policy makers from around Australia.   Participants were selected for their efforts as change agents who have worked proactively to improve the safety of health care in Australia and their desire to further improve safety in health care, in partnership. Participants came together to build trust, functional working relationships grounded in mutual respect and appreciation of what each brought to the field of patient safety and to form strategies and action plans for improving patient safety in Australia. The core of those strategies and action plans is the Perth Declaration for Patient Safety.

    Plans for improving safety in medical care often ignore the patient's perspective. The active role of patients in their care should be recognised and encouraged. Patients have a key role to play in helping to reach an accurate diagnosis, in deciding about appropriate treatment, in choosing an experienced and safe provider, in ensuring that treatment is appropriately administered, monitored and adhered to, and in identifying adverse events and taking appropriate action.

    Today, Patient Safety Learning stands with others around the world to celebrate International Women’s Day 2021. In light of this year’s campaign theme “choose to challenge” we are raising awareness of some of the ways in which male bias can negatively impact on patient safety. Drawing on case studies and quantitative research, this blog focuses on three key areas: Design – using examples to illustrate how male-centric design of equipment and medical devices affects patient safety. Data – discussing how data which does not account for differences between the sexes impacts on patient safety. Dismissal – considering the recurring theme from personal testimonials, and healthcare scandals in recent years, that women’s voices and patient safety concerns are being ignored or dismissed. We will reflect on the key patient safety issues and inequalities in each of these areas and offer our perspective on what needs to happen moving forward to prevent future avoidable harm.

Learning disabilities is a life long condition and they can present in any areas of health care. In this podcast we discover how important it is to have an understanding an appreciation and insight into the care experience of a person with a learning disability and their parents or carers.

    A digital tablet intervention to record and communicate data on the health of residents was used in care homes in Sunderland. Between April 2017 and March 2018, a small-scale evaluation compared data between eight of the care homes routinely using the intervention with eight similar care homes who weren’t. The evaluation found that the eight care homes using the intervention made an estimated saving of around £756,144 in A&E attendances and ambulance services during this period.

    The Covid-19 pandemic has presented new challenges for patients with non-communicable diseases (NCDs) as healthcare systems experience increased resource constraints, conflicting priorities, challenges related to emerging or re-emerging diseases, and difficulties in prioritising NCD services. This report summarises a World Health Organization (WHO) meeting held in December 2020 that aimed to harness the power of community knowledge to tailor priorities, programmes and practices for NCDs and mental health, so that they are realistic, appropriate and attractive to the target populations. Patient experts and representatives with NCDs addressed the following questions: What does meaningful engagement mean? How do we engage meaningfully? Where do we go from here?

    Technology holds promise for the future of healthcare. It can prevent illness, enable early diagnosis, empower health management and support general wellbeing. But how might people use technology to have more control over their health and wellbeing? And do they want to? This report explores the role of technology in managing, improving and supporting health and wellbeing. The NHS Confederation, in partnership with Google Health, commissioned Ipsos to explore people’s behaviours, attitudes and beliefs about responsibility and control when it comes to their health, the role that health technologies play in this and their expectations about the future of healthcare. A survey of more than 1,000 adults in the UK – a third of whom live with one or more long-term conditions (LTCs) – and interviews with individuals with LTCs and who have frequent interaction with the health system, forms the centrepiece of this report.

    A Learning Health System (LHS) is a model of how routinely collected health data can be used to improve care, creating ‘virtuous cycles’ between data and improvement. This requires the active involvement of health service stakeholders, including patients themselves. However, to date, research has explored patients being ‘data donors’ rather than considering patients as active contributors. This study in the journal Health Expectations aimed to understand how patients should be actively involved in a LHS.

Drawing on her own experience as a patient and the founder of a large patient support group, she talks about the invaluable perspective that patients who have experienced healthcare harm can offer policymakers. She also explains why it is important to hear from a wide group of patients who have experienced a variety of issues.

    Measuring a patient’s height is a routine part of a healthcare encounter. But once completed, how often is this information used? For most of us who fall within 95% of the mean population height, this metric is rarely discussed, but what happens when it is overlooked? And what about those on the outer tails of the bell curve of population distribution? Almost 1 million (909,222) adults in the United States are at least 6'4", more than the entire population of South Dakota (884,659). Conversely, an estimated 30,000 Americans have a form of dwarfism, typically defined as an adult height no taller than 4'10". However, despite this prevalence, the healthcare system struggles to provide consistent, adequate care for patients with extreme heights.
