    Poorly designed electronic health records (EHRs) are common, and research shows poor design consequences include clinician burnout, diagnostic error, and even patient harm. One of the major difficulties of EHR design is the visual display of information, which aims to present information in an easily digestible form for the user. High-risk industries like aviation, automotive, and nuclear have guidelines for visual displays based on human factors principles for optimised design. In this study, Pruitt et al. reviewed the visual display guidelines from three high-risk industries—automotive, aviation, nuclear—for their applicability to EHR design and safety.

    In this blog, Clare Crowley and Nick Woodier, National Investigators at the Healthcare Safety Investigation Branch (HSIB) look at the simple but often overlooked measures that NHS staff and organisations can take to improve the design and display of information in the workplace. They refer to a recent HSIB investigation that highlighted how the choice of information to display, and the visibility and accuracy of that information, can influence how NHS staff access and use it.

    Alarms are signals intended to capture and direct human attention to a potential issue that may require monitoring, assessment or intervention. They play a critical safety role in high-risk industries such as healthcare, which relies heavily on auditory and visual alarms. While there are some guidelines to inform alarm design and use, alarm fatigue and other alarm issues are challenges in the healthcare setting. The automotive, aviation, and nuclear industries have used the science of human factors to develop alarm design and use guidelines. This study in the journal Patient Safety aimed to assess whether these guidelines may provide insights for advancing patient safety in healthcare.

    In this video, Michal Seres, who lives with Crohn's disease, talks about his experience of living with an ostomy bag and how he came to develop his own tools to help manage his treatment. Michael established 11 Health, a company which aims to create a collaborative community of patients, healthcare professionals and researchers to develop digital health solutions for patients with chronic illness. Michael talks about the importance of including patients in developing devices and treatments, and how positive, supportive relationships foster collaboration.

    Inadequate medication adherence is a widespread problem that contributes to increased chronic disease complications and healthcare expenditures. Packaging interventions using pill boxes and blister packs have been widely recommended to address the medication adherence issue. This meta-analysis review from Conn et al. determined the overall effect of packaging interventions on medication adherence and health outcomes. In addition, the authors tested whether effects vary depending on intervention, sample, and design characteristics. Overall, meta-analysis findings support the use of packaging interventions to effectively increase medication adherence.

    This chapter from the book 'Managing future challenges for safety' starts with the premise that the future of work is unpredictable. This has been illustrated by the COVID-19 pandemic, and further profound changes in contexts of work will bring significant and volatile changes to future work, as well as health, safety, security, and productivity. Micronarrative testimony from healthcare practitioners whose work has been affected dramatically by the emergence of the pandemic is used in this chapter to derive learning from experience of this major change. The narratives concern the nature of responding to a rapidly changing world, work-as-imagined and work-as-done, human-centred design and systems thinking and practice, and leadership and social capital. Seven learning points were drawn from clinicians’ reflections that may be more widely relevant to the future of work.

    Models and methods of work system design need to be developed and implemented to advance research in and design for patient safety. In this paper, Carayon et al. describe how the Systems Engineering Initiative for Patient Safety (SEIPS) model of work system and patient safety, which provides a framework for understanding the structures, processes and outcomes in health care and their relationships, can be used toward these ends. An application of the SEIPS model in one particular care setting (outpatient surgery) is presented and other practical and research applications of the model are described.

    Over the last 20 years, the Royal College of Art has been a fierce proponent of the role of design to improve and save lives, leading the debate on the efficacy of design thinking when applied to real societal needs. Nowhere is this better exemplified than by its impact on healthcare and patient safety. With increasing pressure on the national healthcare system, public services and provisions have to meet ever more stringent financial, resource and efficiency objectives. The Royal College of Art has demonstrated how systems-led thinking and a design approach to understanding the user’s needs can effectively reduce infection and medical error, and improve treatment spaces and patient communication.

    The COVID-19 crisis has created a watershed moment for the NHS, demanding a reappraisal of how essential services are delivered to the public. Even prior to COVID-19, the NHS recognised a pressing need to rethink healthcare using user-centred design principles, based on populations, not organisations. With the advent of the pandemic that pressing need has become an operational imperative. Digital capability has been and will continue to be a key part of transformation, but will only work when aligned with reforms in other key enablers such as financial flow, workforce planning and regulation. Many industries have already made the shift to enabling collaboration and innovation through more agile models of delivery by embracing technologies like artificial intelligence (AI), internet of things (IoT) and/or flexible and secure forms of (multi) cloud storage. Health, on the other hand, until now has introduced new technologies with the objective of improving existing pathways and service delivery models. There is now an opportunity to reimagine healthcare, driving true transformation enabled by digital capabilities.

    How many times have you tried to plug a USB in the wrong way round? Since both sides are usually similar on the outside, chances are that you have tried to force a USB in upside down at least once. The consequences of this mistake are usually negligible, but in healthcare, poor design can have serious repercussions.  We all know how to plug in a flash drive, so why do we still slip up? Usually, it’s because we are concentrating on something more important. Imagine the same happening in a busy emergency unit where healthcare providers are carrying out life-saving procedures. They’re short on time and working under pressure. One of the paramedics is trying to resuscitate a patient who just went into cardiac arrest — he charges the defibrillator, presses “on”, but instead of shocking, the device switches off, causing the team to waste precious time. What went wrong? And how could the same issue be prevented in the future? Neil Ballinger, head of EMEA at manufacturing parts supplier EU Automation, explains the role of human factors engineering in optimising medical devices.

    Patients with respiratory disease deserve a correct diagnosis and guideline driven care that is standardised, patient focussed and delivered by a healthcare professional with suitable training and experience in a site and timeframe to meet their needs. Sadly, patient groups such as the BLF and Asthma UK have recognised that this is often not the case. The Respiratory Service Framework (RSF) attempts to demonstrate what that excellence is – and how it may be delivered at a population level. Developed by the Primary Care Respiratory Society (PCRS) Service Development Committee, the Respiratory Service Framework helps those looking to design a patient focussed respiratory service working across all sectors of out of hospital care to see the ideal components for a given population of patients. It has been designed to be applicable and helpful to those delivery care at a PCN or ICS level.

    Human-centered design is about understanding human needs and how design can respond to these needs. With its systemic humane approach and creativity, human-centered design can play an essential role in dealing with today’s care challenges. Design’ refers to both the process of designing and the outcome of that process, which includes physical products, services, procedures, strategies and policies. In this Melles et al., address the three key characteristics of human-centered design, focusing on its implementation in health care: (1) developing an understanding of people and their needs; (2) engaging stakeholders from early on and throughout the design process; (3) adopting a systems approach by systematically addressing interactions between the micro-, meso- and macro-levels of sociotechnical care systems, and the transition from individual interests to collective interests.

    What does person centred care mean for the role of patients? Fundamentally, it is an ethos that is intended to put users at the heart of services; encouraging a view of them as participants, not patients; active, not passive. This thinking should apply both to people accessing services as individuals and to citizens collectively – and that means the role of patients, and of their voice, is multiple.     The COVID-19 pandemic has had one of the biggest effects on work-as-done in healthcare in living memory. So what might we learn about work from the perspectives of frontline workers? Steven Shorrock asked a variety of practitioners to give a short answer – whatever came to mind. The themes that emerge centre around people, their activities, their contexts, and their tools. Many insights concerned the varieties of human work, goal conflicts, design, training, communication, teamwork, social capital, leadership, organisational hierarchy, problem solving and innovation, and – generally – change. Steven Shorrock is an interdisciplinary humanistic, systems and design practitioner interested in human work from multiple perspectives.

    User-testing and subsequent modification of clinical guidelines increases health professionals’ information retrieval and comprehension, but no study has investigated whether this results in safer care. Jones et al. compared the frequency of medication errors when administering an intravenous medicine using the current National Health Service Injectable Medicines Guide (IMG) versus an IMG version revised with user-testing. Participants were on-duty nurses/midwives who regularly prepared intravenous medicines. Using a training manikin in their clinical area, participants administered a voriconazole infusion, a high-risk medicine requiring several steps to prepare. They were randomised to use current IMG guidelines or IMG guidelines revised with user-testing.

    A communication passport is a way of supporting a vulnerable person with communication difficulties when they have to transition through different events, such as changing schools, or their first job. Ryan’s family made a specific communication passport for his medical file so that all the medical professionals could learn a little about Ryan before they met him and therefore be better prepared and able to interact with him. Here, his mum shares their example to illustrate how it can be used to improve quality of care.

    This report, published in BMJ Open Quality, sets out the findings of a National Health Service Improvement (NHSI) working group on care communication which included clinicians, patients, patient representatives, NHSI staff and academics from different disciplines. The group’s activities included running four national focus groups and discussion days, in addition to conducting national and international literature searches on healthcare communication and communication improvement.
