    Radar Healthcare has published its 'Incident Reporting in Secondary Care' whitepaper – an in-depth analysis of reporting within secondary care and its effects on patient safety. It has taken a look into the current state of incident reporting: the good work being done, the concerns across the sector, and how we can all aim to improve the situation. The report was conducted using a panel provided by SERMO from its database of UK Nurses and includes the views from 100 nursing staff members working in hospital wards across the UK. Those surveyed work with hospital in-patients daily and are responsible for reporting safety and regulatory incidents involving patients to senior colleagues.

    The number of children and young people admitted to children’s wards with an eating disorder has increased significantly since the start of the Covid-19 pandemic. In the most extreme cases, those with severe malnutrition may need to be fed via a nasogastric tube without their consent. Children’s nurses working on hospital wards may therefore care for children and young people who need to receive nasogastric tube feeding under physical restraint.     Nurses are working the equivalent of one day a week for nothing, according to a study. Researchers from London Economics were commissioned by the Royal College of Nursing (RCN) to look at pay in England, Wales, Scotland and Northern Ireland since 2010. They found that the salary of an experienced nurse had fallen by 20% in real terms, based on a five-day week. Experienced nurses in England, Wales, and Northern Ireland would need a nominal pay rise of 45% by 2024-25 just to return their salaries to levels seen in 2010-11 in real terms, the research said. And such a pay rise would actually help the NHS save money in the long term, as it would be cheaper than hiring staff from overseas, according to the study. Dr Gavan Conlon, who oversaw the research, said that bringing staff in from overseas costs approximately £16,900 more annually than retaining a nurse, while using agency workers costs around £21,300 more per year. He said that about 32,000 nurses leave the NHS every year, many due to the failure of their pay to keep up with the rising cost of living. The RCN is balloting its 300,000 members for strike action, calling for higher pay and an effort by government to fill the hundreds of thousands of nursing vacancies across the country.     A new report published by the NHS AI Lab and Health Education England (HEE) has advocated for training and education for providers in how they deliver and develop AI guidance for staff. The report, entitled ‘Developing healthcare workers’ confidence in AI (Part 2)’, is the second of two reports in relation to this research and follows the 2019 Topol Review recommendation to develop a healthcare workforce “able and willing” to use AI and robotics. It is also part of HEE’s Digital, AI and Robotics Technologies in Education (DART-ED) programme, which aims to understand the impact of advances of these technologies on the workforce’s education and training requirements. In the previous report, the AI Lab and HEE found that many clinicians and staff were unaccustomed to the use of AI technologies, and without the suitable training patients would not be able to experience and share the advantages. The new report has set out recommendations for education and training providers in England to support them in planning, resourcing, developing and delivering new training packages in this area. It notes that specialist training will also be required depending on roles and responsibilities such as involvement in implementation, procurement or using AI in clinical practice. Brhmie Balaram, Head of AI Research and Ethics at the NHS AI Lab, added: “This important new research will support those organisations that train our health and care workers to develop their curriculums to ensure staff of the future receive the training in AI they will need. This project is only one in a series at the NHS AI Lab to help ensure the workforce and local NHS organisations are ready for the further spread of AI technologies that have been found to be safe, ethical and effective.” Read full story Source: Health Tech Newspaper, 25 October 2022

    The NHS faces an “exodus” of female doctors who are struggling to work due to a lack of menopause support, a report has warned. The Medical Protection Society, which helps doctors in legal and ethical disputes, said that many quit or reduce their hours over fears that their menopause symptoms, such as brain fog, insomnia and hot flushes, will cause them to accidentally harm patients. A survey found that 36 per cent of female doctors have considered reducing their hours because of menopause symptoms, while one in five have considered early retirement. “With females making up most of the healthcare workforce, it is crucial that they can access the support they need to avoid an exodus from the profession,” the report said.     A video of an NHS trust’s flamboyant head office complete with a £1,000 coffee machine, sleeping pods and a “great view” has triggered fury from doctors and nurses. Barts Health, which covers hospitals in east London as well as St Bartholomew’s in the City, shared a TikTok video of its corporate office in Canary Wharf. The video, which has since been deleted, showed a luxury coffee machine, “wellbeing rooms” on each floor, free snacks and curved computer screens. However, doctors working at Barts reacted angrily and compared the video with their own facilities. One junior doctor shared a photo of a cramped locker room in the same trust. They wrote on Twitter: “Bags on the floor as ‘no lockers available for juniors’. This tiny room is the entirety of the space available to get changed into mandatory uniform/scrubs – nightmare at shift changeover.” Another shared a picture of their “handmade rest facility” – a row of chairs with paper towels for a pillow. The British Medical Association’s Junior Doctors Committee said it was “sobering” to see the “no expenses spared” approach in the trust’s corporate office.     An ambulance service rated ‘inadequate’ by the Care Quality Commission has set out a wide-ranging improvement plan, including ‘civility training’ for senior leaders and ensuring board members hear a mix of ‘positive and negative’ stories from patients and staff. South Central Ambulance Service has been moved into the equivalent of “special measures” by NHS England, in the wake of the Care Quality Commission report in August which criticised “extreme positivity” at the highest levels of the organisation. This means 3 out of only 10 dedicated ambulance service trusts in England are now in segment four of NHSE’s system oversight framework, the successor to special measures. The other ambulance services in segment four are East of England and South East Coast. In a damning inspection report published in August, the care watchdog said that leaders were “out of touch” and staff had faced a “dismissive attitude” when they tried to raise concerns. One staff member told inspectors: “When sexual harassment is reported it seems to be brushed under the carpet and the person is given a second chance. Because of this, a lot of staff feel unsafe, unsupported and vulnerable when coming to work.” An improvement plan summary published at the start of last month included a large number of priorites and actions, including to “ensure [a] mix of positive and negative patient/staff stories are presented to [trust] board meetings” – an apparent attempt to address CQC concerns that its positive outlook could feel “dismissive of the reality to frontline staff”.     The number of concerns reported by NHS England staff through the freedom to speak up process almost tripled last year, the organisation’s latest board papers have revealed. There were 152 cases received by the internal freedom to speak up guardians in 2021-22 compared to 56 in 2020-21. This year 54 cases were received in quarter three alone. The most common concerns are related to allegations of bullying and harassment. These accounted for nearly 40% of the total. People and team management concerns accounted for a third of FTSU cases. Within the latter, there were sub-themes of breakdown in relationships, failure to offer role models and sanctioning or ignoring poor culture. This week’s report also set out the NHSE FTSU guardian’s next steps. These include appointing a lead guardian, finalising a strategy and continuing to engage with Health Education England and NHS Digital staff as they are brought into NHSE next year.     Nurses will start voting on Thursday on whether to strike over pay amid warnings that record numbers are leaving the profession. Around 300,000 members of the Royal College of Nursing (RCN) are being asked if they want to mount a campaign of industrial action in the union’s first UK-wide ballot. The RCN said new analysis by London Economics to coincide with the ballot launch showed that pay for nurses has declined at twice the rate of the private sector in the last decade. It is the first time in its 106-year history that the RCN has balloted members across the UK on strike action and it is urging them to vote in favour. RCN general secretary Pat Cullen said in a message to those being balloted: “This is a once-in-a-generation chance to improve your pay and combat the staff shortages that put patients at risk. “Governments have repeatedly neglected the NHS and the value of nursing. We can change this if together we say ‘enough is enough’. “Record numbers are feeling no alternative but to quit and patients pay a heavy price. We are doing this for them too."     Both patients and healthcare staff have a central role to play in ensuring the safe use of medicines, Health Minister Robin Swann has said. Minister for Health Robin Swann was speaking at an event to mark the roll out of the ‘Know, Check, Ask’ Campaign across all healthcare sectors in Northern Ireland. The aim of the campaign is to increase awareness and understanding about the importance of using medicine safely. The call for action of the campaign is for: Patients to Know Check Ask – Before you take it: KNOW your medicines and keep an up-to-date list. CHECK that you are using your medicines in the right way. ASK your healthcare professional if you’re not sure. Health Care staff to Know Check Ask – Before you give it: KNOW your medications. CHECK you have the right: patient, medicine, route, dose and time. ASK your patient if they understand and ask your colleagues when you are unsure. Minister Swann added “I want to encourage and help patients to be more curious about their medication, know what medication they are using, how to use it safely and feel able to ask their health care professionals questions about their medicines.
