    A nurse has told how she almost quit her job this month after a patient shouted out racist slurs for hours on a recent night shift. Beverly Simpson, who works as a nurse in a care home in Derry, Northern Ireland, said she was left angry and broken after a patient repeatedly used derogatory racist language and told her ‘to go back to her own country’. Ms Simpson reported the incident on 4 September to managers, who are now investigating. In the meantime, she has called on all employers and peers to do more to protect staff from racism that she says she encounters every week. "I have been a nurse for almost 30 years, but that night made me feel like I wanted to quit,’ she told Nursing Standard. ‘I was just worn down with it all. I’m human, I am hurt, but I still have to go back and treat this patient, be professional and hold my head up high." "It’s all very well a black nurse standing up and saying it is wrong, but we need allies. There needs to be training and protocols on what to do in these situations.     Trainee medics battling Ebola in Uganda's virus epicentre accuse the government of putting their lives at risk. "Most times you come into contact with a patient and you use your bare hands," one worker told the BBC anonymously. All trainees at Mubende's regional hospital say they are on strike and are demanding to be moved somewhere safer. But Ugandan health ministry spokesman Emmanuel Ainebyoona told the BBC there was "no strike at the hospital". Yet all 34 of the hospital's interns - including doctors, pharmacists and nurses - have announced their decision to strike in a joint statement. They say they are being put at undue risk because they lack appropriate safety kit, risk allowances and health insurance.     A coroner has said she does not understand why frontline workers were not required to wear a mask during lockdown after hearing a paramedic had died with Covid. A two-day inquest into the death of Peter Hart, who died on his 52nd birthday, concluded on Tuesday (September 13) with assistant coroner Dr Karen Henderson ruling the father-of-three died of natural causes caused by Covid. She said on the balance of probabilities he caught the disease while working at East Surrey Hospital, where he died on May 12, 2020. During the onset of the pandemic only healthcare workers tending to those suspected of having Covid-19 were required to wear personal protective equipment (PPE). In accordance with national guidelines, Mr Hart, who was treating patients not suspected of having the virus, did not need to. “Retrospectively it is difficult to comprehend why the national guidance said PPE did not need to be used for all patients and healthcare workers at the earliest opportunity,” Dr Henderson said. “Although there appears a lost opportunity to ensure maximum protection I make no finding of fact whether this contributed to Mr Hart’s death. “Patients not suspected to have Covid were not expected to wear face masks. This is in effect a perfect storm and given evidence of Mrs Hart I am satisfied Mr Hart contracted Covid during his work at East Surrey Hospital,” she added.     I guess that a common feature linking most visitors to Patient Safety Learning is that they have a profound interest in two things. First, recognising and applauding innovations and ‘best practice’ in healthcare. Second, recognising, exposing and denouncing bad practice. The thing they have in common is the desire to learn from the mistakes in the past to do better in the future. When it comes to ‘bad practice’ in healthcare it is usually in connection with some adverse and damaging impact on patients. Our thoughts turn perhaps to certain medical failures, such as the ‘Mid‑Staffs scandal’. Seldom do we find the need to consider the adverse and damaging impacts on the doctors, nurses and all the other staff who work in the health and social care sector. However, those of you who watched the recent BBC Panorama programme, 'Forgotten heroes of the Covid frontline' will have been appalled at the scandal that now confronts so many frontline staff for whom we stood outside our front doors and clapped for so enthusiastically back in those dark days at the height of the pandemic. This blog is dedicated to those 'forgotten heroes'. I hope that it demonstrates that they are not, in fact, forgotten I hope that the resources linked to this blog may be of help to them.

    Exposure to ionising radiation during image guided procedures has been associated with a higher incidence of breast cancer in female healthcare workers. Lead or lead equivalent gowns are used to reduce radiation exposure during image guided procedures, but studies have shown that current gowns provide inadequate protection to breast tissue as they leave the upper outer quadrant and axilla exposed.     A number of serious concerns have been raised about the University Hospitals Birmingham NHS Foundation Trust, relating to patient safety, governance processes and organisational culture. The Trust has been under review by the Birmingham and Solihull Integrated Care Board (ICB), following a junior doctor at the trust, Dr Vaishnavi Kumar, taking her own life in June 2022. In response to these concerns, a series of rapid independently-led reviews have been commissioned at the Trust. This report outlines the outcomes of the first of these reviews, which is focused on clinical safety. It identified a number of issues which require attention, setting out 17 recommendations for further action.

She speaks to experts including health psychologist Jodie Eckleberry-Hunt, who highlights that doctors tend to have a lower level of cognitive flexibility, which may affect their ability to cope when things don't go to plan. Psychotherapist Brad Fern goes on to describe the complex range of reasons that doctors may take their own lives, and describes the importance of tackling silence and isolation among doctors.     The Covid-19 pandemic continues to impact heavily on all our lives and one of the long-lasting, but unanticipated, impacts is the emergence of Long Covid. Whilst many people infected by Covid-19 may fully recover, significant numbers will experience varied, ongoing and debilitating symptoms that last weeks, months or years following the initial infection. This prolonged condition has been given the umbrella term Long Covid. Recognition of Long Covid was accelerated by people-led advocacy groups such Long Covid Support. The Office of National Statistics (ONS) reported that, as of 1 August 2021, 970,000 people in the UK were experiencing self-reported Long Covid. The most recent data from 2 January 2023, shows that this has increased to 2 million people This report summarises the findings of a self-selecting survey of 3,097 people with Long Covid in September and October 2022 on their experiences of work.

    In this blog, Patient Safety Learning looks in detail at the results of the NHS Staff Survey 2022, focusing on responses relating to reporting, speaking up and acting on safety concerns. It includes the following key points: It is difficult to imagine other safety critical industries would deem these results acceptable. Nearly half of all respondents did not feel confident their organisation would address their concerns about unsafe clinical practice. It is hugely concerning that over 40% of respondents could not say that they would be treated fairly if involved in a patient safety incident. This could significantly undermine the willingness of staff to raise concerns, with significant consequences for patient safety. There needs to be greater urgency to improve the safety culture in the health service. NHS England needs to recognise the scale of this challenge and provide clarity on how it will work with organisations to tackle this. NHS England, working in partnership with the National Guardian and the Care Quality Commission, should bring forward as a matter of urgency robust and specific commitments to drive forward the work of improving the safety culture in the NHS.

Pressures on general practice are not unique to the UK and GPs around the world are contending with the impact of the pandemic on their patients and working lives. The 2022 Commonwealth Fund survey compares perspectives from GPs across 10 high‑income countries. The survey asked GPs’ views about their working lives and wellbeing, quality of care and how services are delivered.     Second victims are healthcare workers who experience emotional distress following patient adverse events. This mixed method study in BMJ Open looks at how the RISE (Resilience In Stressful Events) programme was developed at the Johns Hopkins Hospital to provide this support. The Need For Recovery (NFR) scale quantifies inter-shift recovery, which contributes to cumulative fatigue and may precede occupational burnout. Advanced clinical practitioners (ACPs) are an established feature of the emergency medicine workforce in the UK, however, little is known about factors affecting their inter-shift recovery, fatigue or how NFR correlates with formal burnout inventories.
