    Doctors are receiving "inadequate" training about the risk of sepsis after a mother-of-five died following an abortion, a coroner has warned. Sarah Dunn, 31, died of "natural causes contributed to by neglect" in hospital on 11 April 2020, an inquest found. Assistant coroner for Blackpool and Fylde, Louise Rae, said Ms Dunn had been treated as a Covid patient even though the "signs of sepsis were apparent". Her cause of death was recorded as "streptococcus sepsis following medical termination of pregnancy". In her record of inquest, the coroner noted Ms Dunn was admitted to Blackpool Victoria Hospital in Lancashire on 10 April 2020. She was suffering from a streptococcus infection caused by an early medical abortion on 23 March, which had produced sepsis and toxic shock by the time she was admitted to hospital. The coroner said "signs of sepsis were apparent" before and at the time of Ms Dunn's hospital admission but she was instead treated as a Covid-19 patient. "Sepsis was not recognised or treated by the GP surgery, emergency department or acute medical unit and upon Sarah's arrival at hospital, the sepsis pathway was not followed," she added.     A private hospital facing a police investigation following a patient’s death has been given an urgent warning by the care regulator due to concerns over patient safety. The Huntercombe Hospital in Maidenhead, which treats children with mental health needs, was told it must urgently address safety issues found by the Care Quality Commission (CQC) following an inspection in March. The CQC handed the hospital a formal warning due to concerns over failures in the way staff were carrying out observations of vulnerable patients. The move comes as The Independent revealed police are investigating the hospital in relation to the death of a young girl earlier this year. In a report published last week, the care watchdog said it had received “mainly negative” feedback from young people at the hospital’s Thames ward, a psychiatric intensive care unit which treats acutely unwell children. Commenting on the hospital overall, the report said: “Young people told us that staff did not follow the care plans in relation to their level of observations. They told us that if there was an incident the staff stopped doing intermittent observations. Staff in charge of shifts on wards asked new staff members to do observations before they understood how to do it. Staff had to ask the young person how to carry out their observations as they did not always understand what was expected of them in carrying out different levels of observations.” Read full story Source: The Independent, 19 May 2022

    More than 27,000 nurses and midwives quit the NHS last year, with many blaming job pressures, the Covid pandemic and poor patient care for their decision. The rise in staff leaving their posts across the UK – the first in four years – has prompted concern that frontline workers are under too much strain, especially with the NHS-wide shortage of nurses. New figures show the NHS is also becoming more reliant on nurses and midwives trained overseas as domestic recruitment remains stubbornly low. In a report on Wednesday, the Nursing and Midwifery Council (NMC) discloses that the numbers in both professions across the UK has risen to its highest level – 758,303. However, while 48,436 nurses and midwives joined its register, 27,133 stopped working last year – 25,219 nurses, 1,474 midwives and 304 who performed both roles. That was higher than the 23,934 who did so during 2020 after Covid struck, and 25,488 who left in 2019. Andrea Sutcliffe, the NMC’s chief executive, said that while the record number of nurses and midwives was good news, “a closer look at our data reveals some worrying signs”. She cited the large number of leavers and the fact that “those who left shared troubling stories about the pressure they’ve had to bear during the pandemic”.     Junior doctors have been prevented from returning to scandal hit heart surgery unit previously criticised over “toxic” culture, The Independent has learned. A coroner defended cardiac surgery at St George’s University Hospital, criticising an NHS-commissioned review into 67 deaths that warned of poor care. However, The Independent has learned the unit received a critical report from Health Education England (HEE), the body responsible for healthcare training, just last year. The NHS authority was so concerned about culture problems and “inappropriate behaviour” within the unit that it took away the junior doctors working there. This is the third time HEE has intervened since 2018, when the unit was criticised in an independent review for having a “toxic” culture. In a statement, Professor Geeta Menon, postgraduate dean for South London at Health Education England, said: “HEE carried out a review of cardiac surgery at St George’s University Hospital in July 2021 and concluded that further improvements were required to create a suitable learning environment for doctors in training. "Unfounded’ NHS criticism and investigation caused unnecessary deaths at London heart surgery unit “We continue to work closely with the trust to implement our requirements and recommendations and will reassess their progress this summer. HEE is committed to ensuring high quality patient care and the best possible learning environment for postgraduate doctors at St George’s.” The Independent understands that a report issued in December, following the HEE visit, identified problems of “inappropriate behaviour”, poor team working from consultants and raised concerns the culture problems previously identified at the unit persisted.     Thousands of patients have been left without vital healthcare after nearly 1 in 10 physiotherapists was prevented from practising after their regulator removed them from its register. Exactly 5,311 physiotherapists were deregistered by the Health and Care Professions Council (HCPC) on 1 May because they had not renewed their registration after the HCPC decided not to send out reminder letters. Ash James, director of practice and development at the Chartered Society of Physiotherapy (CSP), said its helpline had been swamped with calls from distressed physiotherapists, concerned for their patients and worried about dramatic losses in income. “In one of the trusts in Liverpool, 23 physios were sent home in one day, and obviously the implication for patients is huge,” he said. “At a time when the workforce is stretched by the Covid backlog, it’s obviously not ideal that we’ve lost 9% of the workforce overnight.” Physiotherapists have many roles but play a crucial part in helping people leave hospital after long stays, because lengthy bed rest leads to muscle wastage that leaves patients needing physiotherapy to learn to walk again. So far, only about 2,300 physios have been re-registered. With most practitioners seeing at least five patients a day, the number of cancelled NHS and private appointments in the past two weeks could range between 50,000 to 100,000.     Jeremy Hunt has been accused of ignoring serious NHS staff shortages for years and driving medics out of the profession while health secretary after he intervened this weekend to warn of a workforce crisis. Promoting his new book, 'Zero: Eliminating Unnecessary Deaths in a Post-Pandemic NHS', Hunt said tackling the “chronic failure of workforce planning” was the most important task in relieving pressure on frontline services. Now the chair of the health and social care committee, he said the situation was “very, very serious”, with doctors and nurses “run ragged by the intensity of work”. But his comments drew sharp criticism from healthcare staff, who said Hunt – the longest-serving health secretary in the 74-year history of the NHS – failed to take sufficient action to boost recruitment while in the top job between 2012 and 2018. Instead, critics said, his tenure saw health workers quit the NHS in droves for jobs abroad or new careers outside medicine. There are now 100,000 vacancies in the NHS, and the waiting list for treatment has soared to 6.4 million. “There’s an avalanche of pressure bearing down on the NHS. But for years Jeremy Hunt and other ministers ignored the staffing crisis,” said Sara Gorton, the head of health at Unison, the UK’s largest health union. “The pandemic has amplified the consequences of that failure. Experienced employees are leaving at faster rates than new ones can be recruited.” “Hunt has recently been an articulate analyst of current issues, particularly workforce shortages, but these haven’t come out of the blue,” said Dr Colin Hutchinson, the chair of Doctors for the NHS. “At the time he could have made the greatest impact, his response was muted. We have to ask: was the service people were receiving from the NHS better, or worse, at the end of his time in office?     A “shocking” number of nurses from overseas are winding up “in trouble” or sanctioned within their first few months of working in the UK partly because of a lack of induction and support, a conference has heard. The issue was raised during a panel session at the Unison health conference in April discussing the importance of ethical recruitment practices in nursing and midwifery. According to Unison, it is supporting “many” overseas nurses who have been “exploited, unfairly treated and subject to racism” since their move. Among the panel was Gamu Nyasoro, a clinical skills and simulation nurse manager in the NHS and an elected member of Unison’s nursing and midwifery occupational group committee. Ms Nyasoro, who is from Zimbabwe and has been working in the NHS for the past two decades, said she herself had been discriminated against and had faced several challenges during her migration. She raised concern that overseas nurses were not given enough information about how to live and work in the UK, including about how to access healthcare services themselves, or about country specific rules and regulations. There was also the issue that UK employers “don’t look at their skills beforehand”, which means nurses were being put in roles or areas they were not confident in. She cited examples of staff who had been specialising in neonatal services before moving, who were then being asked to work with older people, and those who had been practising as a midwife in their home nations and then being required to work in emergency departments in the UK.     The culture at a long-troubled ambulance trust is ‘worsening, not improving’, its staff have told a health watchdog. Concerns about culture and patient safety at East of England Ambulance Service Trust (EEAST) were raised to inspectors at the Care Quality Commission (CQC) during an inspection of the trust last month, according to public documents. In a feedback letter to the trust following the inspection, the CQC said staffing at EEAST’s control room was below planned levels, and the inspectors were “not assured that staffing levels met the demands within the service and this may impact on patient safety when managing the high volume of calls”. The trust, which is in the equivalent of special measures and currently rated “requires improvement” by the CQC, has had long-standing cultural problems and last year signed a legal agreement with the Equality and Human Rights Commission on how it would protect staff from sexual harassment. According to the feedback letter, staff described a “worsening, not improving, culture” and said the workforce was “tired” and not receiving mandatory training, one-to-ones with managers or appraisals. The letter, published in the trust’s latest board papers, also reported inspectors raising concerns about potential risks to patients over the management of the trust’s call stack and a lack of consistency over “standard operating procedures”. Additionally, some staff in the control room on an accelerated training programme were unable to undertake full patient assessments and had to call for assistance from others.     A baby died after maternity staff repeatedly missed chances to intervene to save his life, an official investigation has found. Giles Cooper-Hall was just 16 hours old when he died after a catalogue of errors in the maternity care of his mother, Ruth Cooper-Hall, at Derriford hospital in Plymouth. A Healthcare Safety Investigation Branch (HSIB) report into the incident has exposed how inexperienced and overstretched staff failed to carry out proper checks, recognise there was an emergency or seek help from senior doctors until it was too late. It comes just weeks after the independent Ockenden report into more than 1,800 cases revealed serious failings in the maternity care provided at Shrewsbury and Telford hospital NHS Trust. It revealed how Ruth Cooper-Hall, then aged 37, was not personally seen by a consultant when she went into labour in October last year, despite recommendations made in the interim Ockenden report published in December 2020. The HSIB report also suggested Giles’ death could have been avoided if staff had known about the care plan for his mother’s labour. Instead, vital messages were not passed on, with the investigation finding this was likely to be because the staff responsible were “distracted” by other tasks.     A coroner has expressed ‘serious concern’ after a trust-wide safety review – prompted by the death of a young mother – was delayed by up to nine months due to ‘staff holidays’. An inquest heard that 25-year-old Natasha Adams, who died by suicide in August 2021, had had her level of care downgraded by Birmingham and Solihull Mental Health Foundation Trust a month earlier, in July, something her family suggested had a “dramatic impact”. She was moved from a “care programme approach” (known as CPA, which involves enhanced care for people with complex needs and/or safety concerns) to “care support” (a non-clinical programme for people with lower-level concerns and complexities). An earlier investigation into her death by the trust, finalised in December, said the trust should audit other cases to check whether the trust’s 2019 “care management and CPA/care support policy” was being complied with. Now Birmingham and Solihull coroner James Bennett has criticised a delay in carrying out the trust-wide audit – writing in a prevention of future deaths report that, as of last month, four months after the report investigating Ms Adams’ care was completed, “no action has been taken”.     NHS bosses in England are urging hospitals to offer staff more overtime and tempt retired employees back, to help tackle waiting lists. A letter sent by NHS England said tackling the backlog that had grown during the pandemic would require a "number of high impact actions". And many hospitals were already taking innovative approaches to the issue. More than six million people are on waiting lists for treatment such as knee and hip surgery. According to the General Medical Council, 21,000 doctors are due to retire in September. And part of the plan would be to tempt some of those back by offering part-time opportunities to: train students run virtual consultations help with follow-up check-ups. Other measures recommended by NHS England are: removing caps on consultant hours - in some places senior doctors are limited to 40 hours a week - to offer extra shifts where safe offering more bank shifts - overtime opportunities for trust rather than agency staff increasing the use of NHS reservists - members of the public who have signed up to work for the NHS for at least 30 days a year - to help run wards, feed patients and provide support for the Covid-vaccination programme exploring taking simple procedures, such as cataract surgery, out of operating theatres and into other parts of the hospital.     A ‘significant flurry’ of senior doctors will look to retire after a trust rejected a scheme designed to avoid higher taxes on their pensions, according to internal emails. Emails from senior doctors suggest Liverpool University Hospitals Foundation Trust has rejected proposals to introduce “pension recycling”, a scheme endorsed nationally to avoid the tax rules. Government policy changes in 2016 reduced tax relief on higher earners’ pension contributions. This has discouraged clinical consultants from taking on extra shifts, and in some cases, prompted people to retire earlier than planned. Last month, NHS England’s chief executive Amanda Pritchard suggested this was a continuing problem for hospitals as they seek to increase elective activity.     An NHS mental health trust that has been the worst performing in England has been warned it must improve after failing another inspection. Norfolk and Suffolk NHS Foundation Trust (NSFT) has been rated "inadequate" in the latest Care Quality Commission (CQC) report. The CQC said it had served the trust with a warning notice that it had to act on to improve patient care. The trust has been rated "inadequate" on three previous occasions by the health watchdog, as well as being the only one currently within the NHS's improvement regime for not meeting standards. Following the latest inspection, its overall rating was downgraded from "requires improvement" - and three out of five measures assessed by the CQC, for safety, leadership and effectiveness, met its lowest grading. The report said two wards were immediately closed to new patients following a CQC visit in November, after the trust was threatened with enforcement action if urgent measures were not taken. Significant staffing problems, including an annual nurse vacancy rate of more than 17%, were also highlighted. Staff at an adult long stay ward did not complete regular checks on patients supposed to happen every 30 to 60 minutes, which meant they were unaware if somebody needed help for periods of up to seven hours. Inspectors also said there had been a severe deterioration on the trust's inpatient ward for children and young people - the Dragonfly Unit in Carlton Colville, Suffolk.     New responsibilities for doctors regarding their use of social media and tackling toxic workplace behaviours and sexual harassment are among key proposals in the General Medical Council’s (GMC) planned update to its core ethical guidance. The regulator has launched a 12-week consultation on the draft new content of 'Good medical practice', which outlines the professional values, knowledge and behaviours expected of doctors working in the UK. This represents the first major update of the guidance since it first came into effect in April 2013, with the review process launched last year. The GMC said the draft new update follows months of working with doctor, employer, and patient representatives, as well as other stakeholders, and reflects the issues faced in modern-day healthcare workplaces. Included for the first time in the draft new guidance is a duty for doctors to act, or support others to act, if they become aware of workplace bullying, harassment, or discrimination, as well as zero tolerance of sexual harassment. For the first time, the GMC's ethical guidance proposes 12 commitments, including: Make the care of patients my first concern. Demonstrate leadership within my role, and work with others to make healthcare environments more supportive, inclusive, and fair. Provide a good standard of practice and care, and be honest and open when things go wrong. Ensure my conduct justifies my patients’ trust in me and the public's trust in my profession.     The Health and Social Care Committee examines the Government’s progress against its pledges on the health and social care workforce and will be the focus of a new independent evaluation by the Health and Social Care Committee’s Expert Panel. Professor Dame Jane Dacre, Chair of the Expert Panel, said: “We’ll be looking at commitments the Government has made on workforce – the people who deliver the health and social care services we rely on. “We’ve identified a recurrent theme in our evaluations to date – whether in maternity, cancer or mental health services, progress is dependent on having the right number of skilled staff in the right place at the right time. Shortages have a real impact on the delivery of services and undermine achievements. “Our panel of experts will evaluate progress made to meet policy pledges in this crucial area - whether it’s about getting workforce planning right, training, or ensuring staff well-being.” The Expert Panel will focus on three areas: Planning for the workforce – including how targets are set, recruitment, and retention. Building a skilled workforce – including incorporating technology and professional development of staff. Wellbeing at work – including support services for staff, and reducing bullying rates. Four specialists have been appointed for this evaluation, bringing their subject specific expertise and experience. They will work alongside the core members of the Expert Panel in identifying a set of Government commitments on workforce and evaluate progress made against them. The findings will support the work of the Health and Social Care Committee which is carrying out a separate inquiry: Workforce: recruitment, training and retention in health and social care.     Despite workforce being the biggest challenge facing the health service, the Health and Care Bill provides no clarity on the numbers of staff this country needs, says Andrew Goddard in a HSJ article. The Health and Care Bill returned to the Commons this week – as did the question of workforce planning. At the end of the spring term, MPs voted to reject an amendment to the bill which would have required the secretary of state to publish independent assessments of current and future workforce numbers every few years. The following week, the House of Lords – led by Baroness Cumberlege, with support from Baroness Harding, Lord Stevens of Birmingham and other cross-party peers – voted to put a revised version of the amendment back in. This particular game of ping pong about how we should plan the NHS and social care workforce is an important one. Workforce is not only a blindspot in the bill – it is a blindspot in the government’s ambitions for health and care. A lack of staff risks undermining the true potential of the Health and Social Care Levy because there will be too few staff to carry out the additional checks and diagnostic procedures promised. The new diagnostic hubs are to be staffed with existing NHS colleagues. Workforce shortages hampered our response to the pandemic and are already having a significant impact on our response to the backlog. They were also identified in the Ockenden Report as a driving factor in the avoidable deaths of 201 babies. It is concerning then, that despite workforce being the biggest challenge facing the health service, the Health and Care Bill provides no clarity on the numbers of staff this country needs.     A clinical director and several senior managers have written to a trust CEO warning that patients are routinely waiting more than 60 hours to be admitted to a ward from accident and emergency, leaving staff “crying with frustration and anger”. In a letter to executives at Lancashire Teaching Hospitals Foundation Trust, seen by HSJ, the managers say they lack support from the rest of the trust, and claim the emergency department at Royal Preston Hospital has a “never-ending elasticity in the eyes of others”. The letter, dated 30 March, is signed by clinical director Graham Ellis, two unit managers, the specialty business manager, and the matron. It says: “Whilst we have documented our concerns previously the current situation is worse than it has ever been…Our situation is increasingly precarious… “For the past few months we have on a regular basis had more than 50 patients waiting for a bed and that wait being in excess of 60 hours. “This means that at most times there is limited or no space to accommodate newly acutely ill patients causing ambulance handover delays of over four hours and delay in treatment.” Clinicians at Preston have been raising safety concerns about the ED for several years, but the letter is the first time concerns of senior managers have been made public. The letter references research which suggests patients die as a “direct result from long waits in ED”, and says there has been an increase in clinical incidents, pressure sores, detrimental outcomes, and occasions where patients “die without the dignity of privacy”.     The chief executive of a mental health trust grappling with care quality failures has described his anger at ‘disrespectful’ staff who have ‘now had to leave the organisation’. In a message to staff, seen by HSJ, Brent Kilmurray, chief executive of Tees Esk and Wear Valleys Foundation Trust, said a number of staff had “stepped away from our values”. HSJ has heard reports of 12 staff members within the trust’s forensic secure inpatient services being suspended in recent weeks, and some dismissed, after being caught sleeping on shifts and using electronic devices while meant to be observing patients. The reports are unconfirmed, but appear to be referenced in a message sent by Brent Kilmurray on 14 March, which said: “I’m sorry to say, there’s been a handful of people who have stepped away from our values and in doing so have now had to leave the organisation." Mr Kilmurray said the staff were in a “minority” and that when the trust investigated these matters “we have found far more excellent caring practice”.     Operations are being cancelled across England as Covid causes “major disruption” inside the NHS, the country’s top surgeon has said, as doctors and health leaders say the government’s backlog targets look increasingly unachievable. Six million people are on the waiting list for NHS hospital care, including more than 23,000 who have waited more than two years. Boris Johnson said in February that he had launched “the biggest catch-up programme in the history of the health service”, but in the same month he dropped every domestic Covid restriction. Now record-high Covid rates are wreaking havoc with the ability of the NHS to catch up with surgery that was delayed or cancelled before and during the pandemic. More than 28,000 staff are off work every day due to Covid, recent figures show, while more than 20,000 patients are in hospital with Covid, which has dramatically reduced the number of beds and space available for planned surgery patients. “Unfortunately, Covid-19 continues to cause major disruption in the NHS, with high staff absences in recent weeks,” Prof Neil Mortensen, the president of the Royal College of Surgeons of England, told the Guardian. “We have heard that planned surgery is being cancelled again in different parts of the country due to staff being off sick with the virus.     Five thousand nurses at Stanford and Lucile Packard children’s hospital in Stanford, California, are preparing to strike in demand of wage increases, mental health and wellness support, better healthcare benefits, and a focus on hiring and retaining nurse staff. The union has set a strike date for 25 April. Stanford hospital at Stanford University in California has been consistently ranked among the top hospitals in the US by US News, but nurses say high turnover rates, understaffing, and inadequate proposed wage increases and benefits have contributed to high burnout rates. In a survey of union members, 45% of nurses reporting said they intend to leave their job within the next five years. Kathy Stormberg, a nurse in the radiology department at Stanford hospital for 19 years and vice-president of the Committee for Recognition of Nursing Achievement (Crona), blamed the strike on the hospitals’ continued reliance on contractors and its policy of pushing nurses to work overtime amid staff shortages, unfilled vacancies, and difficulties retaining enough nursing staff. “That is not sustainable,” said Stormberg. “Nurses have an overwhelming sense of guilt to work overtime when they are getting texts requesting nurses to come in every four hours on their days off.” In January 2022, a nurse on a contract at Stanford hospital walked out of their shift and killed themself, highlighting the need for better mental health and wellness support services and for improvements to the poor working conditions that nurses have faced through the Covid-19 pandemic. “The working conditions that we have now are just no longer sustainable,” said Leah McFadden, a nurse in Stanford’s surgical trauma unit since October 2019. “Over the last two years, we’re starting to run on empty, we aren’t having a chance to decompress, or even just get away from the hospital as much as we should.” Read full story Source: The Guardian, 13 April 2022

    A trust board has backed the medical director who oversaw the dismissal of a whistleblower in a case linked to patient deaths. Portsmouth Hospitals University Trust told HSJ John Knighton had the full support of the organisation when asked if he faced any censure over the wrongful dismissal of a consultant who raised the alarm about a surgical technique. Jasna Macanovic last month won her employment tribunal against the trust with the judge calling its conduct “very one-sided, reflecting a determination to remove [her] as the source of the problem”. The judgment found that the disciplinary process Dr Knighton oversaw was “a foregone conclusion” and as such had broken employment rules. The nephrologist was twice offered the opportunity to resign with a good reference, once during her disciplinary hearing and again on the day the outcome of that hearing was delivered. The trust told HSJ nothing in the judgment suggested Dr Knighton should face any action about his conduct and none had been taken. It said there were no reasons to doubt his credibility or probity. The trust did not respond when asked if any apology had been offered to Dr Macanovic.     There has been a dramatic fall in morale among midwives across multiple measures within the NHS staff survey. Although general morale deteriorated among most staffing groups in 2021, the results for midwives across numerous key measures have worsened to a far greater degree than average. It comes amid the final Ockenden report into the maternity care scandal at Shrewsbury and Telford Hospitals Trust, which raised serious concerns about short staffing and people wanting to leave the profession. The survey results, published on 31 March, suggest 52% of midwives are thinking about leaving their organisation, up 16 percentage points on the previous year. In comparison, the number of general nurses thinking of leaving was 33%, up just 5 percentage points. Chris Graham, chief executive of healthcare charity the Picker Institute, which coordinates the staff survey, described the midwifery profession as an “outlier” in the 2021 results, in terms of how their experiences compare to other groups and how their responses have changed over time. “Not only do midwives report worse experiences in many areas, but there is evidence of particularly sharp declines in some key measures,” Mr Graham said. “It appears likely that staffing shortages are a major factor here.” Read full story (paywalled) Source: HSJ, 13 April 2022

    A trust which is facing major governance issues is failing to respond to hundreds of complaints properly, with patients and families waiting more than twice as long as the NHS target for responses to their concerns, an external review has found. Cornwall Partnership Foundation Trust, which is subject to regulatory action by NHS England, was found to be “not classifying complaints, concerns and comments accurately”, while staff had “no formal training”, meaning complaints were “not investigated appropriately”. Last year, the trust was embroiled in a governance scandal in which NHSE investigated multiple allegations of finance and governance failings, resulting in the departure of former CEO Phil Confue. Rachel Power, chief executive of the advocacy group Patients Association, told HSJ patient complaints often contain “vital intelligence” on how trusts can improve services and “essential warnings about any area where things might be going wrong”. According to the review, the backlog had stemmed from several factors. These included more work being needed on investigations that had not been thorough enough, and the relevant service teams not responding to enquiries by the complaints team. Additionally, there was a “lack of formal monitoring and review” to ensure complaint points were reported appropriately and consistently, and an “apparent lack of accountability by local teams for complaints” triaged through the trust’s patient liaison and complaints team.     Several large teaching hospitals are among those which saw the steepest declines in the proportion of staff who would recommend the care of their organisation, according to the NHS staff survey results. Norfolk and Norwich University Hospitals Foundation Trust, University Hospitals Birmingham, Liverpool University Hospitals FT and Nottingham University Hospitals Trust saw declines of 12 percentage points or more in 2021 — for the proportion of staff saying they would be happy for a friend or relative to be treated at their organisation. This was double the average drop in the acute sector. In a message to staff, Sue Musson, chair of Liverpool University Hospitals Trust, said about her trust’s overall results: “On behalf of the trust board, I want to apologise to everyone that the experience of working at the trust is so deeply unsatisfactory for so many colleagues. “It would be wrong to suggest that there are quick fixes to these issues. The promise I can give you today is a genuine commitment to listen and learn; we particularly need to understand what would make the difference for colleagues across the trust, recognising that there may well be different answers in different parts of the organisation. “We will seek to learn from the trusts that have demonstrated the best staff experience scores and to implement best practices at pace.     The number of GPs in England has fallen every year since the government first pledged to increase the family doctor workforce by 5,000, a minister has admitted. There were 29,364 full-time-equivalent GPs in post in September 2015, when the then health secretary, Jeremy Hunt, first promised to increase the total by 5,000 by 2020. However, by September 2020 the number of family doctors had dropped to 27,939, a fall of 1,425, the health minister Maria Caulfield disclosed in a parliamentary answer. And it has fallen even further since then, to 27,920, she confirmed, citing NHS workforce data. In the 2019 general election campaign, Boris Johnson replaced Hunt’s pledge with a new commitment to increase the number of GPs in England by 6,000 by 2024. However, Sajid Javid, the health secretary, admitted last November that this pledge was unlikely to be met because so many family doctors were retiring early. Organisations representing GPs say their heavy workloads, rising expectations among patients, excess bureaucracy, a lack of other health professionals working alongside them in surgeries, and concern that overwork may lead to them making mistakes are prompting experienced family doctors to quit in order to improve their mental health and work-life balance. The British Medical Association (BMA) said the figures Caulfield cited showed that the lack of doctors in general practice was “going from bad to worse for both GPs and patients”, and it warned that patients were paying the price in the form of long waits for an appointment. “Despite repeated pledges from government to boost the workforce by thousands, it’s going completely the wrong way,” said Dr Kieran Sharrock, the deputy chair of the BMA’s GP committee. “As numbers fall, remaining GPs are forced to stretch themselves even more thinly, and this of course impacts access for patients and the safety of care provided.” Read full story Source: The Guardian, 11 April 2022
