    For about two decades, the U.S. healthcare system was making strides in improving patient safety, as demonstrated by the reduction of healthcare-associated infections and other complications of care. Though there was still room for improvement, the trends were certainly in the right direction. Since the Covid-19 pandemic began, however, many indicators make it clear that healthcare safety has declined. The public health emergency has put enormous stress on the health care system and disrupted many normal activities in hospitals and other facilities. Unfortunately, these stressors have caused safety problems for both patients and staff. The fact that the pandemic degraded patient safety so quickly and severely suggests that our healthcare system lacks a sufficiently resilient safety culture and infrastructure. The authors of this article in the New England Journal of Medicine believe the pandemic and the breakdown it has caused present an opportunity and an obligation to reevaluate healthcare safety with an eye toward building a more resilient health care delivery system, capable not only of achieving safer routine care but also of maintaining high safety levels in times of crisis.

    Long Covid can be difficult to diagnose, and some people might not realise their symptoms could be linked to prior COVID-19 infection.  There can be more than 200 symptoms. Symptoms can affect anywhere in the body. Symptoms can come and go, and new ones can appear weeks or months later. People may not be aware they had Covid-19: some don’t have symptoms at the time of infection, and test results can be unreliable. Long Covid Support and Long Covid Kids have produced an information leaflet on the symptoms of Long Covid.

    This systematic review published in BMJ Global Health looks to identify the nature, frequency and causes of long-COVID symptoms by reviewing data from existing research studies into long-COVID. It aims to regularly synthesise evidence on long-COVID characteristics to help improve long-term outcomes. From the data provided by 39 studies, the review found that: long-COVID affects both patients who were hospitalised and those managed in the community patients with long-COVID display a wide range of symptoms including weakness, general malaise, fatigue, concentration impairment and breathlessness research currently available on long-COVID is vulnerable to bias, so caution should be used when interpreting data. The authors also identify areas where further research is needed to help define long-COVID symptoms, identify risk factors for different populations and assess the impact of variants of concern and vaccination on long-term outcomes.

He highlights that organisations can take proactive steps to mitigate the consequences of staff being affected by long COVID, such as staff absences and changes to job roles. He recommends that organisations: introduce regular contingency planning activities (such as COVID Resilience meetings) ensure managers are trained to support staff living with long COVID ensure occupational health and staff wellbeing services include support relevant to long COVID consider establishing peer support groups for affected staff.

It aims to describe how children and young people are affected by post-COVID physical symptoms and mental health problems and to identify those most at risk.     To safeguard patient safety and the wellbeing of healthcare staff, a realistic approach to tackling the backlog of non-COVID care is needed. NHS and public health services have been running ‘hot’ for a prolonged period of time and an overstretched and exhausted workforce must now be given time to rest and recuperate as they meet the challenges ahead. If staff are being pushed too hard to restore routine care in an unrealistic timeframe and without suitable resources, the likelihood is that we will see a workforce squeeze due to a combination of increasingly high staff absence rates and staff reducing their hours or leaving the workforce altogether. This would make it harder for health services to get back on track and provide timely and safe care to patients who need it.
