    The aim of the study was to explore the factors that affect the safety attitude and teamwork climate of Cyprus maternity units and Cypriot midwives. The study found that the safety climate in the maternity settings was negative across all six safety climate domains examined. The higher mean total score on team work and safety climate in the more experienced group of midwives is a predominant finding for the maternity units of Cyprus. It could be suggested that younger midwives need more support and teamwork practice, in a friendly environment, to enhance the safety and teamwork climate through experience and self-confidence.

    This report by the Royal College of Midwives (RCM) highlights the impact of midwifery staffing shortages on women. It looks at historical failures to invest appropriately in maternity services and talks about a mounting maternity crisis, drawing attention to Care Quality Commission inspections of maternity services that are identifying concerns around safety directly linked to staffing shortages. According to the report’s findings, if the number of NHS midwives in England had risen at the same pace as the overall health service workforce since the last general election, there would be no midwife shortage; there would be 3,100 more midwives in the NHS, rather than having a shortfall of 2,500 full-time midwives. The RCM published the results of a survey last month which showed that midwives give 100,000 hours of free labour to the NHS per week to ensure safe care for women. It also showed that staffing levels were repeatedly cited as cause for concern around the safety of care, and that midwives and maternity support workers are exhausted and burnt out.

    In March 2019, NHS England published Saving Babies Lives version 2, which included information for providers and commissioners of maternity care on how to reduce perinatal mortality across England. One element of this recommends the appointment of a fetal monitoring lead with the responsibility of improving the standard of fetal monitoring. The aim of the fetal monitoring lead is to support staff working on the labour ward to provide high quality intrapartum risk assessments and accurate CTG interpretation and should contribute to building and sustaining a safety culture on the labour ward with all staff committed to continuous improvement. The importance of fetal monitoring was highlighted again in the Ockenden Report, published December 2020. The report recommended, as an essential action, that all maternity services must appoint a dedicated lead midwife and lead obstetrician, both with demonstrated expertise, to focus on and champion best practice in fetal monitoring. Monitoring May is a month long learning event based around fetal monitoring, human factors, maternity safety and shared learning.     Institutional racism within the United Kingdom's (UK) Higher Education (HE) sector, particularly nurse and midwifery education, has lacked empirical research, critical scrutiny, and serious discussion. This paper focuses on the racialised experiences of nurses and midwives during their education in UK universities, including their practice placements. It explores the emotional, physical, and psychological impacts of these experiences. The study concludes that the endemic culture of racism in nurse and midwifery education is a fundamental factor that must be recognised and called out. The study argues that universities and health care trusts need to be accountable for preparing all students to challenge racism and provide equitable learning opportunities that cover the objectives to meet the Nursing and Midwifery Council (NMC) requirements to avoid significant experiences of exclusion and intimidation.

    Despite being the employees who often have the most direct contact with service users, NHS clinical support workers, such as healthcare assistants and maternity support workers, have long experienced a range of barriers to their effective deployment and development. These include a lack of standardised entry requirements, inconsistent task deployment and truncated career progression pathways. These have a detrimental impact on service delivery, including patient satisfaction. The degree to which local employers are able to determine the recruitment, deployment and development of support workers is a key reason why these issues endure; however, this article suggests that a deeper reason is the existence of a segmented labour market in the NHS, with support workers existing in a secondary market. This duality resides in the socio-economic differences between registered and non-registered staff. Recent NHS support workforce strategies present an opportunity to finally address the issues support staff face.

    There are at least 7,469 research nurses and midwives across the UK and Ireland working within all areas of healthcare, reveals a landmark new census initiated by a group of NIHR 70@70 Senior Nurse & Midwife Research Leaders. The census, incorporating responses from research nurses and midwives across all four UK nations and the Republic of Ireland, reveals nurses and midwives are working at every level in healthcare from Bands 5 – 9 in the UK, and from staff nurse to Directors of Nursing or Midwifery in the Republic of Ireland. This suggests there are opportunities to join the profession at every level, with continued potential for career progression. Clinical research nurses and midwives are a specialist workforce, with knowledge, skills and expertise in both clinical practice and research delivery. The census shows that: 33.7% reported working in joint posts, for example as a clinical research nurse for part of their role as well as a clinical nurse specialist; 72% are working within a single disease/area specialism; 28% reported covering multiple disease areas. NIHR Director of Nursing & Midwifery Professor Ruth Endacott said: “This census reveals the true breadth and depth of our research nursing and midwifery community. We know there are scores of people working incredibly hard day and night helping to bring us new treatments and medicine alongside their healthcare colleagues but we now have a much clearer idea of the size of the workforce.     The NHS could be forced to dismiss almost 2,000 midwives by the government’s mandatory vaccination policy, amid warnings from a former chief nurse of England that mothers and babies will be put at risk. Well-placed senior sources have told HSJ around 1,700 midwives remain unvaccinated nationally, according to the latest data from trusts. Based on official headcount data that would amount to between 6.5-8% of the workforce, depending on whether it counts full time equivalent or total staff numbers. However, they are mostly in London, with the latest estimate in the city said to be about 680 (representing between 12 and 14% of the workforce), several well placed sources told HSJ, meaning its maternity services could be seriously destabilised. A former chief nurse of England, Sarah Mullally, who now sits in the House of Lords as the Bishop of London, said she believed about 12.5% of London’s midwives were unvaccinated, and called on the government to delay the mandatory health worker vaccination policy. Speaking in Parliament yesterday, she warned mothers and babies would be put at risk, “in order to implement a policy that has been superseded by the evolution of the virus”. She added: “I would strongly encourage everyone, including NHS staff and health care workers, to get fully vaccinated. However, having heard from midwives myself this week, I can see the anxiety that the requirement for mandatory vaccination is causing, as well as the potential risks to the heath service and its patients.     Hundreds of nurses, paramedics, health and care workers have been disciplined over allegations of sexual assault, including incidents involving child sexual abuse, The Independent can reveal. It comes as the government begins a year-long inquiry into the sexual abuse of dead patients by “morgue monster” David Fuller. Charities claim the true scale of the issue is likely to be hidden by “vast underreporting” while safeguarding experts say there is no “uniformity” in how NHS trusts handle such cases. The Health and Care Professions Council (HCPC), which regulates just under 300,000 workers including paramedics, occupational therapists, psychologists and physiotherapists, has taken action on 154 occasions following 293 investigations carried out into allegations of sexual assault or abuse since 2012, according to figures obtained by The Independent. Fifty-three clinicians were struck off, 20 were cautioned and a further 29 were either suspended, had restrictions placed on their practice or agreed to be removed from registration. More than half of the actions followed allegations of sexual abuse of a child patient.     A trust has warned it could be forced to restrict maternity services due to a high midwife vacancy rate, and large numbers unvaccinated among the current staff. The government has mandated that all patient-facing NHS staff must have had two covid vaccination doses from 1 April — meaning they will need to have received their first dose by 3 February. If not, they can be redeployed to non patient-facing roles, or face dismissal. Barking, Havering and Redbridge University Hospitals Trust’s board heard on Tuesday that the current numbers pose a “significant operational problem” amid efforts to encourage more staff to get both covid jabs before the government’s deadline. The board meeting was told that, of the trust’s 7,550 staff, approximately 1,300 workers – or 17.4% – do not have a vaccination recorded against them, with the areas of greatest concern being women’s and children’s health, geriatric services, the emergency departments and some clinical support services. At the board meeting, BHRUHT chief executive Matthew Trainer said: “The vacancy rate, plus the unvaccinated rate, would put us in quite a serious position. “At the minute, for example, the Queen’s Birth Centre [at Queen’s Hospital in Romford, east London], I don’t think, has been open since I got here. I couldn’t see any circumstances in which it would reopen if we lost another chunk of midwives, for the foreseeable future certainly, in terms of vaccination. “I think it would leave us in a position where we’d have to look at constraining services and focusing in on core [services], establishment being focused on the labour ward, looking at complex births and making sure we’re doing everything we possibly can to manage it as safely as possible.” Read full story (paywalled) Source: HSJ, 12 January 2022

    A severe shortage of midwives has led to home birth services being closed or reduced by a number of hospital trusts across the UK, with pregnant women frequently left in limbo as to where they will be able to give birth. The Observer has found more than 20 trusts that have had disrupted home birth services in the past three months. Eight confirmed their services remain suspended due to staff shortages. They include East Kent Hospitals, Swansea Bay University Health Board and NHS Dumfries and Galloway – all of which report that the situation is under constant review. Home birth services at some trusts, such as Walsall Healthcare NHS Trust and University Hospitals of Derby and Burton, have been closed since August. Others have reopened after short suspensions or have written to expectant parents to say they cannot guarantee sending a midwife when there is high demand or staff shortages. The findings come a week after midwives across Britain staged protests to call on the government to address the “crisis” in maternity care, with staff suffering from chronic burnout and stress. Midwives are being driven out of the NHS by understaffing and fears they cannot deliver safe care, according to a recent survey published by the Royal College of Midwives (RCM). Maria Booker, programmes director at the charity Birthrights, said: “Staffing pressures in maternity services are very real right now. But for many women the option to give birth at home is not a luxury but the only option that feels safe to them." “Some know they will labour better at home while some do not want to visit hospital during a pandemic.     Midwives across England are still not receiving enough essential safety training with the pandemic leaving hospitals delivering less training than three years ago. A new report from the charity Baby Lifeline, based on an investigation of 124 NHS trusts in England, found 9 in 10 units had training affected by the pandemic with staff shortages named as a major factor in preventing workers from taking time out for learning. This was cited by 72% of units as a problem. The average spend on maternity training was significantly lower in 2020-21 at £34,290 compared to £59,873 in 2017-18, with NHS trusts delivering less training to staff than they did in 2017-18. Despite concerns over the poor quality of safety investigations in the NHS, fewer than a third of NHS units trained staff in how to carry out investigations. Judy Ledger, chief executive and founder of Baby Lifeline, said: “Today’s report highlights how gaps and variation in the delivery of maternity training across the NHS continues to impact on the safety and care women and babies receive. Time and again evidence shows that training investment can save lives, and the pandemic has widened existing, detrimental gaps that years of chronic under-funding and staff shortages have created.     The Royal College of Midwives is calling for members to be given the same support as doctors when they struggle with drug and alcohol problems. Research shared with the Guardian – the first of its kind into substance abuse among midwives – has revealed that significant numbers of midwives have problems with drugs or alcohol. It found that 28% said they had problems and 16% said they worked while under the influence of various substances. Dr Sally Pezaro, the author of the research, found that along with alcohol, midwives used cannabis, cocaine, heroin and sedatives. She said reasons given include work-related stress and anxiety, bullying, traumatic clinical incidents and to maintain overall functioning. Pezaro, a midwife and fellow of the Royal College of Midwives, said there was no dedicated support programme available for midwives, while doctors struggling with substance abuse can get support from the NHS practitioner health programme. The incidence of substance abuse among midwives, based on the survey of 623 people, appears higher than among doctors – 8%-15% of whom have been found to have substance abuse problems. Pezaro is calling for more support for midwives. “If midwives seek help they lose their licence to practise. Doctors are treated less harshly by their regulator, the General Medical Council, than we are by ours – the Nursing and Midwifery Council,” she said. Between 2014 and 2016 26% of 1,298 fitness to practise cases that came before the Nursing and Midwifery Council (NMC) involved drugs or alcohol.     A whistleblowing letter sent by maternity staff to inspectors and a newspaper was "the right thing to do", the hospital's boss said. Midwives at West Suffolk Hospital in Bury St Edmunds said they were "exhausted and broken" and claimed the unit was "consistently short-staffed". The hospital had previously been criticised for its treatment of whistleblowers. Its interim chief executive Craig Black said the letter was a "brave thing". The anonymous letter was sent to the Bury Free Press, the Care Quality Commission (CQC) and the West Suffolk Foundation NHS Trust, in August. It claimed the midwives had spoken out because standards of care had fallen sharply. Staff were "under extreme pressures all the time, which has left them fed up, exhausted and burnt out", it said.     The trust at the centre of a maternity scandal does not have enough midwifery staff to keep women and babies safe, a Care Quality Commission (CQC)inspection has revealed. East Kent Hospitals University Foundation Trust relied on community midwives to fill slots at its acute unit, with some of them working 20-hour days after being called in to help cover and feeling outside of their competence. The trust had suspended a midwife-led unit and diverted women in labour to other hospitals – and when the CQC raised the understaffing issue at its inspection in July, it suspended its home birth service. But the CQC found that the number of midwives and maternity workers on duty rarely matched planned numbers and managers rarely calculated staffing numbers accurately, with some elements of the workload not being factored in. Lack of staff meant there was a risk to the safe assessment and monitoring of women and babies at the trust’s William Harvey Hospital in Ashford.     More than three-quarters of midwives think staffing levels in their NHS trust or board are unsafe, according to a survey by the Royal College of Midwives (RCM). The RCM said services were at breaking point, with 42% of midwives reporting that shifts were understaffed and a third saying there were “very significant gaps” in most shifts. Midwives were under enormous pressure and had been “pushed to the edge” by the failure of successive governments to invest in maternity services, said Gill Walton, the chief executive of the RCM. “Maternity staff are exhausted, they’re demoralised and some of them are looking for the door. For the safety of every pregnant woman and every baby, this cannot be allowed to continue,” she said. “Midwives and maternity support workers come into the profession to provide safe, high-quality care.     NHS maternity units have been told they have until next April to increase the numbers of midwives on wards to expected levels after a near £100 million investment. NHS England has told hospitals they must bring staffing levels for midwives up the levels needed to meet their planned demand from mothers and to ensure women get safe care. In a letter to NHS trusts, England’s chief nurse Ruth May said she expected hospitals to use their share of a recent £96 million investment by NHS England to boost staffing levels along with extra spending from local budgets. NHS England has carried out an analysis of demand and supply with Health Education England as part of a four year plan to boost the number of midwives. Hospitals are expected to set the level of midwives needed to deliver more one-to-one care and to try and ensure more than half of women see the same midwife throughout their pregnancy.     Maternity services are at risk because demoralised midwives are planning to quit the NHS, healthcare leaders have warned. A new report, carried out by the Institute for Public Policy Research, suggests 8,000 midwives may depart due to the “unprecedented pressure” of the coronavirus pandemic. Researchers, who surveyed about 1,000 healthcare professionals from around the country in mid-February, discovered that two-thirds reported being mentally exhausted once a week or more.     The Royal College of Midwives (RCM) has launched a new positioning statement to call for a Digital Midwife in every maternity service in the next 12 months. The trade union, which represents the majority of practising midwives, has called for every trust to recruit or train Digital Midwives to lead on digital transformation programmes and ensure systems that are introduced are interoperable. The RCM has said it’s not just a call for investment but a need to ‘drive forward digital transformation and clinical informatics of maternity care’. Hermione Jackson, RCM Digital Advisor, said: “For too long maternity services have been overlooked, passed over and generally left at the back of the queue when it comes to digital investment. Investing in digital technology and giving staff the training and equipment they need will lead to better care, regardless of where that care is delivered. “There is clear evidence that more and better use of digital technology is supported by women, midwives, maternity support workers and other maternity staff. We need the Government and hospital Trusts and Boards to give maternity services the tech they need to do their jobs even better. Improvements have been happening but at a snail’s pace and we need to see this move much more rapidly simply to catch-up with other areas of the NHS.” The RCM said it will be publishing new guidance on electronic record keeping for midwives and maternity support workers later in March.     Midwives at Suffolk Hospital have spoken out in a whistleblowing letter describing problems in their department as ‘demoralising and heartbreaking’. In the letter, written by midwives who declined to give their names "for fear of retribution", describe constant staff shortages, a culture of blame and fear, a high pressure environment and substandard care, saying " We entered midwifery to be able to give women centred, holistic care. Instead it feels like we are being overwhelmed by the unmanageable and relentless workload, and as a result are giving substandard care which is demoralising and heartbreaking. We are all feeling like we are now desperate for change. This change is beyond what we can achieve ourselves so we urge you to please help us to generate it. It should not be accepted or tolerated for us to be forced into giving unsafe care entirely due to unsafe staffing". In response, Karen Newbury, head of midwifery at the trust, said: “We are working exceptionally hard to recruit additional midwives and we are very grateful for the flexibility and dedication of our staff in ensuring that we provide a safe and caring service – this was recognised by our Care Quality Commission inspection in April which found we managed safety well. We have recently completed recruitment so there will be at least two senior midwives on every shift to provide flexible and experienced support to our maternity teams, and we are working with colleagues regionally to recruit staff both locally and internationally as well as running a full student training programme.” Read full story.     In the wake of the Nottingham Hospital maternity scandal, the hospital is now trying to find 70 midwives to fill vacancies. In recent years, concerns about staff shortages and patient safety has been raised, with staff even writing a letter to the trust board over their fears. A spokesperson from the trust has said “We will endeavour to continue recruiting until all vacancies have been filled, and our staff will continue working tirelessly to improve services for local women and families.” Read full story.     New NHS pelvic health clinics have been set up to help and support thousands of pregnant women and new mothers who are experiencing incontinence and other issues related to the pelvic floor. Women receiving care at 14 new pilot sites will be treated throughout their pregnancy. Among the treatment, women will learn how to perform pelvic floor exercises with a physiotherapist as well as receive advice on diet with continued support and monitoring throughout. Read full story.
