    People in England are struggling to get dental treatment, as dentists close to new NHS patients, a watchdog says. Healthwatch England, the NHS body representing patients, said the problem was made worse by the rising cost of living and needed "urgent attention". It said some people were living in pain, unable to speak or eat properly, because they could not find treatment. And it warned the poorest were suffering most as they were least able to afford to pay for private dentistry. Healthwatch England said the issue was creating a two-tier system - dividing the rich and the poor - and called on the government to take action. "There is now a deepening crisis," said Louise Ansari, of Healthwatch England. "With millions of households bearing the brunt of the escalating living costs, private treatment is simply not an option - and even NHS charges can be a challenge. "This needs urgent attention."     New calculations from Cancer Research UK estimate that, on average, over 65,000 people in England are left waiting longer than 28 days to find out whether they have cancer each month. These estimates are based on the latest data from the Faster Diagnosis Standard (FDS). The FDS is a performance standard introduced by Government in 2021. It’s used to better capture how long people on certain cancer-related referrals wait for a diagnosis. This applies to people referred by their GP urgently with suspected cancer, following breast symptoms, or have been picked up through cancer screening. The current FDS target is set at 75%, meaning three quarters of people being urgently referred should be told they have cancer or given the all-clear within that timeframe. However, this target has yet to be met. In addition, the data has revealed major variation across the country – with only 78 of 143 trusts meeting the 75% target. This means that despite the tireless efforts of NHS staff, chronic capacity issues mean that people continue to be failed by the system. Michelle Mitchell, our chief executive, said: “As a country we should not be willing to accept that over 1 in 4 people on an urgent referral are left waiting over a month to find out whether they have cancer. Nor should we stand for the variation that exists across the country.” The charity is calling on Government to include a more ambitious target within its upcoming 10-year cancer plan, to help ensure around 54,300 more people each month receive a diagnosis or have cancer ruled out within a month. With a robust plan and sustained investment to build a cancer workforce fit for the future, patients will be diagnosed quicker and earlier, which will save more lives.     A 94-year-old man has said his GP refuses to see him “unless it’s life or death”. Dennis Baker, from North Hampshire, said he felt “put off” by his doctor's surgery, which is a three-minute walk from his house. The pensioner, who lives with his wife who has advanced dementia and is bed-bound, said he found it “quite difficult to carry on a conversation with a doctor” and cannot get one to visit him at home. “The chances are [the receptionist] will say… ‘you're not dying, a doctor will phone you at some stage today’, that’s the usual response,” he told BBC Radio 4’s World at One. It comes as the president of the Royal College of GPs (RCGP) said family doctors should start “saying no” to extra work to tackle the crisis in primary care. Speaking at Pulse Live last week, Professor Dame Clare Gerada said the workload crisis was not the fault of GPs and they “cannot innovate [their] way out”. “When you’re in debates and people are saying to you 'you’ve got to work harder and smarter' - no, the rest of the system has to adapt,” she told the conference. “You have to start saying no.” Read full story (paywalled) Source: The Telegraph, 3 May 2022

    More than 38,000 patients were put at risk of harm during March – more than 4,000 of them seriously – while they waited in an ambulance outside hospital, according to estimates shared with HSJ. The number of hour-plus delays to handing over patients from ambulances to emergency departments in March was the highest ever recorded, following steep increases since last summer. Figures collected by the Association of Ambulance Chief Executives (AACE), and shared with HSJ, reveal that one trust recorded a delay of 23 hours during March. Based on its detailed information about the length of handover delays, AACE has produced an estimate of the likely number of patients harmed while waiting to he handed over, using a model initially developed in research published last year. This found 85 per cent of those who waited more than an hour could have suffered potential harm. The AACE report said that patients who waited the longest were at greatest risk of some level of harm and the risk of severe harm tripled for those waiting for more than four hours compared with waiting for 60 to 90 minutes. AACE managing director Martin Flaherty told HSJ: “We expect the situation to be no better when we collate our figures for April. “The most significant problem remains hospital handover delays which continue to increase exponentially, with tens of thousands of ambulance hours being lost due to hospital handover delays, causing enormous knock-on effects out in the community, where delays in people receiving the ambulance resource they need are the obvious result. “However, the human cost, in terms of direct harm that is being caused to patients through these combined delays at hospitals and in the community, as well as to the health and wellbeing of our ambulance crews, is substantial."     An NHS boss who had a stroke was taken to A&E by her husband rather than calling for an ambulance because of concerns over long waits. In a series of tweets, Gloucestershire Hospitals NHS Trust chief executive Deborah Lee praised his swift actions. She said he had "bundled her into his car", last week, after she had showed the signs of a stroke because he had heard her "lamenting ambulance delays". She is recovering but says it may have been different if they had called 999. Waits for an ambulance in England are the longest since new targets were introduced, in 2017. And Ms Lee's regional service - the South West - has the longest waits in the country, with category-two calls, which include strokes, taking nearly two hours, on average, to reach patients in March. The target is 18 minutes. In the tweets, Ms Lee said: "Naturally, I am eternally grateful to my husband for his swift actions… but I can't get one thing out of my head. "What if my husband hadn't been there and my daughter had called for an ambulance and I'd been put in the cat[egory]-two stack?" She went on to say it was not the fault of the ambulance service and the whole system was "working unrelentingly to this but to no great avail". Ms Lee said hospitals were struggling to discharge patients, because of a lack of social care, and so delays were building up in the rest of the system.     A shortage of specific types of hormone-replacement therapy (HRT) has left women struggling with untreated symptoms of the menopause. Demand for prescriptions has more than doubled since 2017 - partly because of work by campaigners to extend access - but supply of some products has not kept up. Yasmin Darling's experience of the menopause was sudden and profound. Two years ago, she had two operations to reduce her risk of inherited cancer which plunged her into an early menopause. "It's really hard to navigate medical menopause 10 years early," the 45-year-old says. "When you don't have the product you need, it makes it much more difficult to navigate." Because of Covid, Yasmin has never been seen by a specialist at a menopause clinic, so she is managing as best she can on her own. Claire Lopez, 59, spent three weeks trying to obtain her usual HRT patches from different local chemists but they were out of stock. Without them, her body becomes "very stiff", leading to slipped discs and severe back pain. "I have severe anxiety if I do not have these patches, so the total lack of coordination between GPs and pharmacists was extremely frustrating," Claire says. In the end, she had to arrange a private prescription through a local clinic, in the Midlands, costing £50. The government has said it is determined to ensure supplies of HRT can meet high demand. Minister for Women's Health Maria Caulfield said: "There are over 70 HRT products available in the UK, most of which remain in good supply. "However, we are aware of some issues with women being unable to access certain products. "We will be appointing a new HRT-supply chairperson and convening an urgent meeting of suppliers to look at ways we can work together to improve supply."     At least 200,000 people missed out on essential surgery as a result of the Covid-19 pandemic, with many enduring “misery and daily pain” as a result, a conference has heard. Both scheduled and emergency surgery levels dropped by 20% during the pandemic, suggesting there is now significant pent-up demand for treatment, according to the national clinical lead in surgery, Prof Deborah McNamara. Almost 343,000 people are waiting to see a surgeon for the first time, 100,000 of whom have been on a waiting list for more than 18 months, she told the conference on outcomes from the pandemic at the Royal College of Surgeons in Ireland (RCSI). This was only the start of delays for patients, she pointed out, as they have to wait again for their procedure to be carried out. Currently, more than 71,000 patients are waiting for surgery, a fifth of whom have been on the list for more than a year. Long-waiting patients needing complex surgery have been disproportionately affected, she said, as the system focused on treating “quick-win” procedures such as endoscopies. The amount of day-case work carried out by hospitals is back to 84 per cent of 2017 levels, yet complex care remains at only 67 per cent, she pointed out. Patients waiting for surgery were enduring a “huge amount of misery” that remains unquantified, according to Prof McNamara. The pandemic resulted in some positive changes, she said, including shorter hospital stays, a greater role for physician associates and a generational change in the use of IT.     Women are being left unable to sleep or work competently because of the shortages of hormone replacement therapy (HRT) products used to treat symptoms of the menopause, the former cabinet minister, Caroline Nokes, has said. Millions of women go through the menopause every year, with many experiencing symptoms that can be severe, such as low mood, anxiety, hot flushes and difficulty sleeping, and have a negative impact on everyday life. The number of prescriptions for HRT in England has doubled in the last five years to more than 500,000 a month. But the rise in prescriptions has come amid several years of HRT shortages, with pharmacists often unable to fulfil prescriptions. Shortages have been blamed on manufacturing and supply issues, and have been exacerbated by the growing numbers of women seeking the products. Speaking in the Commons on Thursday, Nokes, chair of the women and equalities committee, called for an urgent debate on the issue to ensure women “can get the supplies that we need”. In October, the government announced that the cost of repeat prescriptions for HRT would be significantly reduced in England. In the Commons on Thursday, Labour MP Nick Smith asked Spencer why there was “no date yet for the HRT prescription changes in England”.     A hospital for adults with eating disorders has been rated inadequate after inspectors found the provision of food was "unsafe and unacceptable". A Care Quality Commission (CQC) report of the Schoen Clinic in York said some patients were given mouldy bread and one was served food containing plastic. Concerns were also raised around lack of staff and patient safety, though wards were clean and well-equipped. Schoen Clinic Group said issues raised in the report "were quickly addressed". Following the inspection in January the hospital has been placed in special measures and will be visited again in six months. Brian Cranna, CQC's head of hospital inspection, said: "The standards of care we found at Schoen Clinic York were putting patients at risk and so we have taken urgent enforcement action, which means the service must improve if it's to retain its registration."     A woman has described how she spent more than six hours of her 100th birthday waiting in agony for an ambulance after slipping and fracturing her pelvis while getting ready for a family lunch. Irene Silsby was due to be picked up by her niece, Lynne Taylor, for a celebration to mark her centenary on 9 April. But she fell in the windowless bathroom of her care home in Greetham, Rutland, and staff called an ambulance at 9am after she managed to summon help. “All I remember is I was in terrible pain,” said Silsby from her hospital bed on Saturday. When asked of the ambulance delay, she said: “It’s disgusting. I don’t know how I stood it so long, the pain was so severe.” Taylor expected to meet the ambulance as she arrived 45 minutes later. But when she reached the care home, the manager said it would be a 10-hour wait, she said. What was to be her aunt’s first trip outside the care home in more than five months turned into her lying on a cold floor surrounded by pillows and blankets to keep her warm and quell some of the discomfort. Taylor, 60, recalled her aunt saying: “They’re not coming to me because they know I’m 100 and I’m not really worth it any more.” Taylor said she had never felt so scared, frustrated and worried. After calling 999 and expressing her outrage, she was told that life-threatening conditions were being prioritised. “I thought she was going to die,” she said. “I didn’t think that any frail, tiny, 100-year-old body could put up with that level of pain on the floor.” Read full story Source: The Guardian, 20 April 2022

    Ambulance trusts are seeing rising numbers of serious incidents resulting from delays in reaching patients, research by HSJ has uncovered. Serious incidents are defined by the NHS as a patient safety failure “where the consequences to patients, families and carers, staff or organisations are so significant or the potential for learning is so great, that a heightened level of response is justified.” East Midlands Ambulance Service Trust saw 71 serious incidents in 2021-22 compared with 38 in the financial year before. The trust’s board papers attribute the increase in SIs related to delayed responses since June 2021 to “sustained pressure on the service” and the resulting growing handover times at accident and emergency departments. Of 14 SIs reported in February and the first half of March 2022, seven were due to “prolonged waits for an ambulance response”. West Midlands University Ambulance Service Foundation Trust has also seen an increase in SIs. Its board papers report that half of the SIs are due to “delays in reaching patients resulting in harm, serious harm, and deaths”. It has given the issue of “hospitals, breaches, delays and turnaround times” the maximum rating of 25 on its risk register. Long delays – especially for category two patients, where average performance last month was above an hour – are causing increasing concern. Stroke Association chief executive Juliet Bouverie said the organisation was hearing “shocking accounts from stroke survivors who have waited hours for an ambulance… We are extremely worried that stroke survivors’ lives and recoveries are being put at extreme risk.” Read full story (paywalled) Source: HSJ, 20 April 2022

    Growing numbers of patients in the UK are paying for private medical treatment because of the record delays people are facing trying to access NHS care, a report has revealed. They are using their own savings to pay for procedures that involve some of the longest waiting times in NHS hospital, such as diagnostic tests, cataract removals and joint replacements. The increase in the willingness to self-pay is closely linked to a desire for private treatments that was increasing even before Covid struck in March 2020. But many private hospitals were unable to meet that demand for much of the pandemic because coronavirus disrupted so much normal healthcare. Dr Tony O’Sullivan, an ex-NHS consultant and a co-chair of the campaign group Keep Our NHS Public, said: “The government’s deliberate and sustained running down of the health service has resulted in a two-tier system. The NHS is now in a permanent state of distress, leaving patients desperate for care, and – if they can afford it – feeling as if they have no choice but to go private, undermining the very vision of equality and care a well-funded NHS was so famous for. “Hard-working people would not need to line shareholders pockets in this way if the NHS had not been underfunded, understaffed and neglected for so long.” Read full story Source: The Guardian, 20 April 2022

    Thousands of lives are being put at risk due to delays and disruption in diabetes care, according to a damning report that warns patients have been “pushed to the back of the queue” during the Covid-19 pandemic. There are 4.9 million people living with diabetes in the UK, and almost half had difficulties managing their condition last year, according to a survey of 10,000 patients by the charity Diabetes UK. More than 60% of them attributed this partly to a lack of access to healthcare, which can prevent serious illness and early mortality from the cardiovascular complications of diabetes, rising to 71% in the most deprived areas of the country. One in three had no contact with healthcare professionals about their diabetes in 2021, while one in six have still not had contact since before the pandemic, the report by the charity said. Diabetes UK said that while ministers have focused on tackling the elective surgery backlog, diabetes patients have lost out as a result, and there is now an urgent need to get services back on track before lives are “needlessly lost”. Chris Askew, the chief executive of Diabetes UK, called for a national diabetes recovery plan. “Diabetes is serious and living with it can be relentless,” he said. “If people with diabetes cannot receive the care they need, they can risk devastating, life-altering complications and, sadly, early death. “We know the NHS has worked tirelessly to keep us safe throughout the pandemic, but the impacts on care for people living with diabetes have been vast.     Volunteers will transport patients who need extra assistance to hospital to increase ambulance availability for higher-risk patients. The pilot scheme, using ambulance cars, is due to start in London in May. London Ambulance Service said the trained volunteers would be sent to lower category 999 calls where the patient needed help to get to hospital. The service's board meeting was told the scheme would reduce waiting times and increase ambulance availability. Currently, taxis are used to transport "low acuity patients" to hospital, the meeting heard. But there were some patients who required "the assistance of one person to walk or mobilise", which taxis could not provide. "This results in the dispatch of an emergency ambulance, reducing ambulance availability for higher priority incidents and longer waiting times for patients," the meeting heard. As part of the pilot scheme, a volunteer car would be dispatched to these patients. A spokesperson for the service said: "This project builds on our well-established network of volunteers who respond to emergencies to help ensure our ambulances can reach the patients that need us the most. "These fully trained volunteers, who already respond to 999 calls in their communities, will help patients who have been assessed not to need of an ambulance but who may need more support than a taxi can provide."     Operations are being cancelled across England as Covid causes “major disruption” inside the NHS, the country’s top surgeon has said, as doctors and health leaders say the government’s backlog targets look increasingly unachievable. Six million people are on the waiting list for NHS hospital care, including more than 23,000 who have waited more than two years. Boris Johnson said in February that he had launched “the biggest catch-up programme in the history of the health service”, but in the same month he dropped every domestic Covid restriction. Now record-high Covid rates are wreaking havoc with the ability of the NHS to catch up with surgery that was delayed or cancelled before and during the pandemic. More than 28,000 staff are off work every day due to Covid, recent figures show, while more than 20,000 patients are in hospital with Covid, which has dramatically reduced the number of beds and space available for planned surgery patients. “Unfortunately, Covid-19 continues to cause major disruption in the NHS, with high staff absences in recent weeks,” Prof Neil Mortensen, the president of the Royal College of Surgeons of England, told the Guardian. “We have heard that planned surgery is being cancelled again in different parts of the country due to staff being off sick with the virus. This is understandably frustrating for surgical teams who want to help their patients by getting planned surgery up and running again.     Five thousand nurses at Stanford and Lucile Packard children’s hospital in Stanford, California, are preparing to strike in demand of wage increases, mental health and wellness support, better healthcare benefits, and a focus on hiring and retaining nurse staff. The union has set a strike date for 25 April. Stanford hospital at Stanford University in California has been consistently ranked among the top hospitals in the US by US News, but nurses say high turnover rates, understaffing, and inadequate proposed wage increases and benefits have contributed to high burnout rates. In a survey of union members, 45% of nurses reporting said they intend to leave their job within the next five years. Kathy Stormberg, a nurse in the radiology department at Stanford hospital for 19 years and vice-president of the Committee for Recognition of Nursing Achievement (Crona), blamed the strike on the hospitals’ continued reliance on contractors and its policy of pushing nurses to work overtime amid staff shortages, unfilled vacancies, and difficulties retaining enough nursing staff. “That is not sustainable,” said Stormberg. “Nurses have an overwhelming sense of guilt to work overtime when they are getting texts requesting nurses to come in every four hours on their days off.” In January 2022, a nurse on a contract at Stanford hospital walked out of their shift and killed themself, highlighting the need for better mental health and wellness support services and for improvements to the poor working conditions that nurses have faced through the Covid-19 pandemic. “The working conditions that we have now are just no longer sustainable,” said Leah McFadden, a nurse in Stanford’s surgical trauma unit since October 2019. “Over the last two years, we’re starting to run on empty, we aren’t having a chance to decompress, or even just get away from the hospital as much as we should.” Read full story Source: The Guardian, 13 April 2022

    NHS leaders are warning that the health service is facing the "brutal reality" of an Easter as bad as most winters. Latest data shows record waits for planned surgery and in A&E, as staff plough through a backlog fuelled by Covid. The government says there is hope on the horizon. Jean Shepherd, 87, had a stroke in April last year, leaving her severely disabled and requiring round-the-clock care. At the end of February there was an outbreak of sickness at her nursing home and she needed hospital treatment. She had to wait in a wheelchair for more than 9 hours until an ambulance arrived to take her to A&E. She then spent 31 hours on a trolley between the emergency department and a secondary-care unit. "She was very distressed because she doesn't like hospitals at the best of time," says her son, Andy Shepherd. "Since the stroke, because of her cognitive ability, she doesn't understand what's happening around her." Mrs Shepherd was eventually moved to a bed in a main hospital ward, where her family says she later contracted Covid, before recovering and being discharged back to her care home two weeks later. "I appreciate that A&E departments have always been busy, but I just wasn't prepared for what greeted me at the hospital," says her son. "There were patients on ambulance trolleys literally everywhere and the staff were absolutely rushed off their feet. I remember thinking at the time that this is not sustainable."     There has been a dramatic fall in morale among midwives across multiple measures within the NHS staff survey. Although general morale deteriorated among most staffing groups in 2021, the results for midwives across numerous key measures have worsened to a far greater degree than average. It comes amid the final Ockenden report into the maternity care scandal at Shrewsbury and Telford Hospitals Trust, which raised serious concerns about short staffing and people wanting to leave the profession. The survey results, published on 31 March, suggest 52% of midwives are thinking about leaving their organisation, up 16 percentage points on the previous year. In comparison, the number of general nurses thinking of leaving was 33%, up just 5 percentage points. Chris Graham, chief executive of healthcare charity the Picker Institute, which coordinates the staff survey, described the midwifery profession as an “outlier” in the 2021 results, in terms of how their experiences compare to other groups and how their responses have changed over time. “Not only do midwives report worse experiences in many areas, but there is evidence of particularly sharp declines in some key measures,” Mr Graham said. “It appears likely that staffing shortages are a major factor here.” Read full story (paywalled) Source: HSJ, 13 April 2022

    More than four hours after an ambulance was called, Richard Carpenter, 71, who had had a suspected heart attack, began to despair. “Where are they?” he asked his wife, Jeanette. “I’m going to die.” She tried to reassure her husband that the crew must surely be close. Perhaps they were struggling to find their rural Wiltshire home in the dark. “But I could see I was losing him,” she said. She gave her husband CPR and urged him: “Don’t leave me.” But by the time the paramedics arrived another hour or so later, it was too late. Jeanette Carpenter, 70, a stoical and reasonable person, accepts it might have been impossible to save her husband. “But I think he would have had more of a chance if they had got here sooner,” she said. It is the sort of sad story that is becoming all too common. Across England, but in particular in the south-west, ambulances are too often not getting to patients in a timely manner. Before Covid, said one ambulance worker – who asked not to be named – he would do between six and 10 jobs in a shift. Now if the first person he is called to needs to go to hospital, he expects this will be his one job for the whole shift. “At some hospitals we are waiting outside hospitals for 10, 11 or 12 hours,” he said. “There’s nothing more demoralising than hearing a general broadcast going out for a cardiac arrest or road accident and there’s no resources to send.     Two years ago the first wave of the covid pandemic reached its peak. The NHS had reacted with impressive speed to prepare for an influx of patients with an infectious disease that few knew much about, had no cure for, and for which there was no known vaccine. However, now the NHS goes into the Easter break in a more fragile state than in any previous winter since, at least, the 1990s. This is not just the direct result of covid hospitalisation, of course – although the distracting narrative of ‘with rather than because of covid’ has obscured how hugely damaging any kind of infectious disease that is as widespread in the community as covid is now can be to effective hospital care. For someone who has just undergone an operation, for example, the greatest threat is not from catching covid itself, but from the impact the virus may have on how quickly their wound may heal. Perhaps covid’s greatest continuing impact is on growing staff absences and the pernicious impact it is having on the long-term health of those who had the disease – even in some cases where it has been relatively mild. For the tens of thousands who have been hospitalised with covid, the consequences for their long-term health look more serious every day. Much of this new workload is ending up at the doors of primary and community care – and displacing other needs and services just when they are most required after two years of coping with the pandemic. There is usually one thing you can confidently say about the NHS, which is that in any crisis it will make sure the life-saving decisions are made on time. However, in the South West, and probably other regions too, that is not happening. People are dying because the NHS cannot – despite its best efforts – save them.     Long waiting times at Devon’s acute hospitals have forced commissioners to offer patients treatment 200 miles away in London in a bid to reduce the elective backlog. Devon Clinical Commissioning Group has secured extra capacity for patients requiring complex orthopaedic surgery under a new deal with the South West London Elective Orthopaedic Centre, located at Epsom General Hospital. The NHS-run orthopaedic centre is around 170 miles from Exeter in east Devon and 210 miles from Plymouth in west Devon. Many patients have declined to go, despite the CCG offering to cover their travel costs. It is the longest publicly reported distance patients are being sent for elective treatment in the NHS, with patients usually referred to neighbouring hospitals or integrated care systems if there is no capacity at their local provider. Nearly 1,500 patients in the Devon ICS have waited longer than two years for treatment.     NHS chiefs have issued an extraordinary plea for families to help them discharge loved ones even if they are Covid-19 positive as the health service faces a “perfect storm” fuelled by heavy demand, severe staff shortages and soaring Covid cases. Hospitals and ambulance services across England are under “enormous strain”, health leaders have warned, after NHS trusts covering millions of patients declared critical incidents or issued stark warnings to residents. Dr Layla McCay, director of policy at the NHS Confederation, which represents the whole healthcare system, said the situation had become so serious that “all parts” of the health service were now becoming “weighed down”. This will have a “direct knock-on effect” on the ability of staff to tackle the care backlog, she added, as well as the current provision of urgent and emergency care. On Wednesday evening, the crisis became so acute in Hampshire and the Isle of Wight that its chief medical officer urged relatives of patients well enough to be discharged to collect them immediately – even if they were still testing positive for coronavirus. Dr Derek Sandeman, of the Hampshire and Isle of Wight Integrated Care System, revealed that almost every hospital in the two counties was full, and said the number of people with Covid-19 being cared for in hospitals across the area was 650 – more than 2.5 times higher than in early January. He added that 2,800 staff working for local NHS organisations were off sick, half of which absences were due to Covid-19. “With staff sickness rates well above average, rising cases of Covid-19 and very high numbers of people needing treatment, we face a perfect storm – but there are some very specific ways in which people can help the frontline NHS and care teams,” said Sandeman.     Patients in nine hospitals in Ireland were often treated in the wrong places, sometimes corridors, in situations where it was “unclear” who was supposed to be providing their care, a clinical review has found. It warned of the potential for people to receive inappropriate specialist input and recommended specific wards be used to avoid so-called “safari rounds” where consultants must seek out scattered patients. The independent review team consisted of clinical and management experts from Scotland and England who undertook a programme of visits between August and November, 2019. “The review team witnessed widespread boarding and outliers – any bed, anytime, anywhere and including mixed gender,” the document said. “This does not create extra capacity, leads to safari rounds, increases length of stay, introduces harm by non-specialist care and increases staff absenteeism.” Although acknowledging often excellent work by staff, the report was commissioned to examine non-scheduled care at nine hospitals found to be “under the greatest pressures” during the winter season of 2018/2019.     The NHS in England is struggling to make progress on its flagship target to diagnose three-quarters of cancer cases at an early stage, MPs are warning. The Health and Social Care Committee said staffing shortages and disruption from the pandemic were causing delays. Some 54% of cases are diagnosed at stages one and two, considered vital for increasing the chances of survival. By 2028, the aim is to diagnose 75% of cases in the early stages, but there has been no improvement in six years. It means England - as well as other UK nations - lag behind comparable countries such as Australia and Canada when it comes to cancer survival. If the lack of progress continues, the committee warned that it could lead to more than 340,000 people missing out on an early cancer diagnosis. The Department of Health said it recognised "business as usual is not enough" and said it was developing a new 10-year cancer plan.     Stroke and heart attack victims are now routinely waiting more than an hour for an ambulance, after a further fall in performance in recent weeks, and with hospital handover delays hitting a new high point, HSJ reveals. Figures for ambulance performance this week, seen by HSJ, showed average response times for category two calls at more than 70 minutes for successive days. 3,000 patients may have suffered “severe harm” from delays in February, ambulance chief executives say. Several well-placed sources in the sector said response times had deteriorated further this month, and that more than half of ambulance trusts were this week seeing average category two responses of longer than an hour. Some cited an average category two response last week of around 70 minutes, with the services under huge pressure from a combination of demand, long handover delays, and covid-related sickness. Category two calls include patients with suspected heart attacks and strokes, and the national target for reaching them is 18 minutes. The figures seen by HSJ for this week showed average response times for category one calls — the most serious, including cardiac arrests and other immediately life threatening emergencies — of more than 10 minutes on Wednesday, against a target of just 7 minutes.
