    With record-long waits for treatment, it has never been so important for NHS trusts to understand the level of risk to patients on the waiting lists. But while it’s one thing to assess and categorise the patients and their risks while waiting, it’s quite another to then subsequently intervene to effectively care for patients during that wait. With the use of technology, there are potentially enormous gains to be made on waiting list management, and one integrated care system is forging ahead on this front. The ICS in question is Cheshire and Merseyside. HSJ takes a look at the progress Cheshire and Merseyside are making.

In April 2022, Amanda Pritchard requested a review of the way in which the NHS, working in partnership, delivers effectively on its current priorities while developing the culture and capability for continuous improvement.     On 1 July 2022, Integrated care systems (ICSs) were placed on a statutory footing. ICSs are partnerships of organisations that come together to plan and deliver joined up health and care services, and to improve the lives of people who live and work in their area. Following their introduction, on the 18 November 2022, the Government announced that it would commission an independent review into the oversight of ICSs, considering how to best enable them to succeed, balancing greater autonomy and robust accountability, to be led by former Secretary of State for Health, the Rt Hon Patricia Hewitt. This report sets out the findings of this review.

    The NHS Patient Safety Incident Response Framework (PSIRF) was launched in 2022 and is intended for full implementation by Autumn 2023. PSIRF requires Integrated Care Board (ICB)’s to work collaboratively with providers to develop a Patient Safety Incident Response Plan (PSIRP) and Patient Safety Incident Response Policy. Within the PSIRP, each organisation must work with their ICB and other stakeholders to identify how it will respond proportionately to all incidents requiring investigation.  Suffolk and North East Essex NHS Foundation Trust share their Standard Operating Procedure on PSIRF ICB sign off process.

    Major new reform of the NHS will not work until Government addresses multiple chronic issues in the service, says the Public Accounts Committee (PAC) in a new report. The case has not been made for what improvements Integrated Care Systems (ICSs) will bring to patients, and by when.  ICSs are the latest attempt to bring NHS and local government services together to join up services and focus on prevention. But the Committee says the reforms will founder if the major systemic problems in the NHS are not addressed by Government at a national level:  the elective care backlog has breached seven million cases for the first time; major workforce issues have hamstrung both the NHS and social care; constantly increasing demand; a crumbling NHS estate; and limits on funding.   These challenges require national leadership but there is a worrying lack of oversight in the new system, and crucial national projects like the NHS Workforce Plan and capital funding strategy are repeatedly delayed – what the Committee calls 'paralysis by analysis'. The cost of overdue maintenance has reached £9 billion - £4.5 billion classed as high or significant risk - and there are questions about who gets to keep proceeds of any assets sold under ICSs.    Not enough is being done to focus on preventing ill-health, and not enough joint working between government departments to tackle the causes of ill-health. The failure to ensure adequate NHS funded dental care risks creating more acute dental health problems.

    The health service’s independent data watchdog has issued a warning to local NHS bodies over concerns confidential patient information is being shared unlawfully with third parties, including for ‘population health’ analysis. In a letter to integrated care systems (ICSs), National Data Guardian Nicola Byrne and UK Caldicott Guardian Council chair Arjun Dhillon said they had both “been made aware that within some local record sharing programmes, organisations could be processing confidential patient information without ensuring that the processing does not breach confidentiality”. They added among the four areas of concern health and care staff had raised with them was that confidential patient information may be being transferred from local record sharing programmes to third party hosted secure data environments. Secure Data Environments are data storage and access platforms where organisations can apply to access data for planning and research purposes.     At least half of integrated care systems (ICS) lack plans for responding to cyberattacks, at a time of increasing cyber risks, HSJ can reveal. The findings also come at a time when the threat posed by cyber attackers is “constantly evolving”, and in the wake of a recent high-profile attack on a supplier to several trusts. In August 2021, NHS England published a framework – What Good Looks Like – to set out what ICSs and member organisations must achieve to be considered digitally mature. Requirements included that all ICSs should have a system-wide plan for “maintaining robust cybersecurity” with “centralised capabilities to provide support across all organisations”. However, 20 ICSs have told HSJ they do not yet have such a cybersecurity strategy or plan in place. Nine ICSs said they did, while the remaining 13 ICSs did not respond. This is despite the NHS being subjected to a growing number of cyber attacks. In 2020-21, NHS Digital reported the health service had been targeted roughly 21 million times on a monthly basis, which marked an increase since before the pandemic. Most of these are malicious emails containing malware and are automatically blocked by cyber defence and monitoring systems. However, in August, a dozen mental health trusts and several NHS 111 and urgent care providers were badly affected by a cyber attack on one of their IT suppliers, Advanced.     There are too many integrated care systems (ICSs) in some parts of the country, especially the South West and Midlands, writes Alastair McLellan and Dave West in this HSJ article. This means that effective integration will struggle due to limited resources, leadership capacity and ability to influence large providers. It is also a problem widely acknowledged at the centre and within the regions.  The proposed structure for ICSs is overly complex, consisting of a partnership board with little statutory power, which is meant to give strategic direction to an executive board which in practice will be held accountable for all decisions, but which is also meant to give up as much power and money as possible to “place based” entities which remain ill defined and have no statutory standing.

    Plans to establish integrated care systems (ICSs) as statutory bodies in the health and care bill foreshadow further changes to the organisation of the NHS. Unlike previous reorganisations, the changes expected to occur in 2022 have developed from within the NHS rather than being imposed by the government. Not only this, but leaders in the NHS have also played a major part in shaping the nature of these changes in partnership with the centre.  This paper from the NHS Confederation focuses on the changes needed to create the conditions in which ICSs can improve outcomes for patients and the public and outlines a series of simple rules to guide those leading the reform programme. The ideas put forward are intended to provide a basis for debate with healthcare leaders and others in England about next steps. The paper starts from the premise that a key role of leaders is to harness the intrinsic motivation of health and care staff and public health teams to perform to the best of their abilities. The distinctive contribution of ICSs is to work with partners in making use of all available assets and leading improvements in patient care and outcomes that require actions across the organisations and services that make up the health and care system.     Integrated care systems (ICSs) are geographically based partnerships that bring together providers and commissioners of NHS services with local authorities and other local partners to plan, co-ordinate and commission health and care services. They are part of a fundamental shift in the way the health and care system is organised – away from competition and organisational autonomy and towards collaboration, with health and care organisations working together to integrate services and improve population health. ICSs have been developing for several years – the Health and Care Bill will put them on a statutory footing from April 2022.

Integrated care systems (ICSs) need to recognise the role communities can play in improving and sustaining good health, and as part of this they need to seek greater involvement with local voluntary, community and social enterprise (VCSE) groups at the place and neighbourhood level, where the link local communities is at its strongest. This conference will provide an opportunity to discuss the impact of community-led and person-centred approaches to improving health and wellbeing, and to explore what more can be done to build on community interventions, assets and solutions that developed as a response to the pandemic.     The focus of GGI’s April health and care leaders webinar is systems partnerships and the Hewitt report, in conversation with Rt Hon Patricia Hewitt, a former Secretary of State for Health and now a Chair of an ICB. The Hewitt Report on systems partnerships and governance will really frame how boards will lead during this next crucial year for the NHS. With an election next year, all NHS board members will be navigating their way through financial and performance pressures of the likes never seen before. How the new ICBs behave will impact every single NHS board. In a ‘Chatham House Rule’ session this will be a valuable chance to understand the forces at play and what individual NHS directors need to understand in terms of context and direction.
