    The chairman of Covid vaccine giant AstraZeneca has said that investment in technology can help the NHS cut costs. Leif Johansson said more spending on areas such as artificial intelligence and screening could prevent illness and stop people going to hospital. The NHS is under severe pressure, with A&E waits at record levels and strike action exacerbating ambulance delays. Mr Johansson said about 97% of healthcare costs come from "when people present at the hospital". He said only the remaining 3% is made up of spending on vaccination, early detection or screening. Mr Johansson told the BBC at the World Economic Forum in Davos: "If we can get into an investment mode in health for screening or prevention or early diagnostics on health and see that as an investment to reduce the cost of sickness then I think we have a much better model over time that would serve us well." Commenting on the UK, he said: "All countries have different systems and the NHS is one which we have learned to live with and I think the Brits, in general, are quite appreciative about it."     Woodside Care Village in Warwick is staged like a town centre in miniature, with benches and a fountain, cafe tables and front doors to homes styled as either “town”, “country” or “classical”. But none of the places are quite what they seem, because here everything has a greater purpose: to improve the wellbeing of people with dementia. Modelled on a groundbreaking Dutch experiment in looking after people with Alzheimer’s disease, the purpose-built facility, which opened in 2019, is quietly breaking new ground for a better kind of dementia care. “Everything is dressed and staged to look familiar,” said Jo Cheshire, the communications manager for the home’s operator, WCS. “We try to make sure people aren’t severing their links with the past. We have one lady who works in the launderette with a badge, because that’s what she did before. It feels like they are contributing to the community.” “The idea is you have freedom,” said Cheshire. “If you come upon a locked door it can increase agitation, that’s unsettling for the other residents and it makes the carer’s job harder.” Staff ratios are higher than normal, at two staff for every five or six people rather than the usual one. This means staff can spend more time interacting with the residents. Staff are briefed with a “this is me” document, which details the likes and dislikes of each person with dementia and has photos through their lives, the time they like to get up, when they like to eat. A clinical trial of such “person-centred” dementia care in 69 care homes in London and Buckinghamshire published in 2020 showed that it improved quality of life for people with dementia and reduced agitation and the burden of depression or aggression.     The reform of the UK’s Medical Device regulation offers a golden opportunity to drive innovation and growth in the UK’s Life Science sector while ensuring patient safety remains at the heart of the regulatory approach. But there is an urgent need for action to ensure we do not lose this opportunity. Senior members of the Life Sciences Council, Will Quince MP, Minister of State at the Department of Health and Social Care, Dr June Raine, CEO, Medicines and Healthcare products Regulatory Agency (MHRA) and Peter Ellingworth, CEO, Association of British HealthTech Industries (ABHI) have today announced a new agreement to accelerate the delivery of the future UK HealthTech regulatory system. Acknowledging the Chancellor’s priorities of stability and growth supported by regulatory reforms, and the importance of the success of the system to UK patients and the Life Science sector, they have formed an advisory group on behalf of the Life Sciences Council to drive the delivery of the ambition of the Life Sciences Vision to have a best in class regulatory system. The advisory group has agreed that aligned proposals will be published on three priority areas: international recognition routes for innovation system capacity.     A soft robotic device that moves through the body like a worm could make bowel cancer screening much more comfortable by reducing pain and and discomfort, according to the scientists developing it. Researchers at Imperial College London said they have created a new type endoscope that is soft, flexible, and capable of extending and curling on its own inside the body. Around 900,000 colonoscopies – which involves an endoscope being passed through the bottom – are performed every year in the UK, the majority of which are for screenings for bowel cancer. The procedure is invasive and more than 75% of patients develop significant pain, according to Nisha Patel, a consultant gastroenterologist at Imperial College Healthcare NHS Trust in London, who is running clinical trials with the device. She said: “We know patients experience either discomfort or pain during the procedure and this affects uptake of further procedures and patient experience. The experts are also hoping that compared to current endoscopes, which requires specialist skills, their self-propelling robotic version will be easier to use and could, in future, be deployed in GP surgeries or outpatient clinics.     Pioneering new technology could help patients with non-healing wounds avoid infections and the need for antibiotics, scientists say. Wirelessly powered, environmentally friendly “smart bandages” have been developed by a team of scientists from the UK and France, with the University of Glasgow and the University of Southampton leading the research. The bandage could help improve the quality of life for people with chronic non-healing wounds as a result of conditions such as cancer, diabetes or damaged blood vessels, they said. Currently, wounds require painful cleaning and treatment. Researchers believe the technology could help to slow the rise of dangerous new strains of antibiotic resistant bacteria known as superbugs.     Investors are pouring billions into companies claiming they can analyse DNA to find the disease early. But some scientists question if they really work. A pioneering group of people in the US and UK who have elected to take part in a new form of cancer screening known as multi-cancer early detection tests (MCED). The tests use gene sequencing or other novel technologies to detect fragments of DNA expelled by cancerous cells which circulate in people’s blood, allowing the identification of multiple types of cancer from a single blood draw. They have been hailed as “revolutionary” and “cutting edge” by British and US health chiefs. Health bodies in both nations have set up MCED clinical trials in the hope that the tests can be rolled out to the population at large. The UK’s NHS is participating in a clinical trial of the Galleri test involving 140,000 patients. But not everyone is convinced the tests live up to the hype. Several health experts and scientists told the Financial Times that the tests could harm rather than help some patients due to risks associated with misdiagnosis, over-diagnosis and over-treatment.     A breakthrough AI model can determine a person's risk of developing pancreatic cancer with staggering accuracy, research suggests. Using medical records and information from previous scans, the AI was able to flag patients at a high risk of developing pancreatic cancer within the next three years with great accuracy. There are currently no full-proof scans for pancreatic cancer, with doctors using a combination of CT scans, MRIs and other invasive procedures to diagnose it. This keeps many doctors away from recommending these screenings. Over time, they also hope these AI models will help them develop a reliable way to screen for pancreatic cancer — which already exists for other types of the diseases. "One of the most important decisions clinicians face day to day is who is at high risk for a disease, and who would benefit from further testing, which can also mean more invasive and more expensive procedures that carry their own risks," Dr Chris Sander, a biologist at Harvard who contributed to the study, said. "An AI tool that can zero in on those at highest risk for pancreatic cancer who stand to benefit most from further tests could go a long way toward improving clinical decision-making."     GPs in South London have eradicated a large gap in blood pressure control between white patients and those from a Black or minority ethnic background, after a year-long project. AT Medics Streatham PCN in Lambeth found that among their patients under 80 diagnosed with hypertension, there was a 12% inequality gap in blood pressure control, with 67% of white patients and 55% of black patients treated to target. The two practices in the PCN have 45,000 patients, with around 3,100 diagnosed with hypertension. They set up a centralised recall system and made use of practice pharmacists and healthcare assistants to provide guidance, education around self-care, lifestyle and medicines. Dr Tarek Radwan, GP director said: ‘This project has delivered incredible results, and this is all down to the dedication of our amazing team, especially our administrators, healthcare assistants and pharmacists. "The last 12 months have proved that we can not just reduce but actually eradicate health inequalities and raise the quality of care for everyone at the same time.     Doctors, scientists and researchers have built an artificial intelligence (AI) model that can accurately identify cancer in a development they say could speed up diagnosis of the disease and fast-track patients to treatment. Cancer is a leading cause of death worldwide. It results in about 10 million deaths annually, or nearly one in six deaths, according to the World Health Organization. In many cases, however, the disease can be cured if detected early and treated swiftly. The AI tool designed by experts at the Royal Marsden NHS foundation trust, the Institute of Cancer Research, London, and Imperial College London can identify whether abnormal growths found on CT scans are cancerous. The algorithm performs more efficiently and effectively than current methods, according to a study. The findings have been published in the Lancet’s eBioMedicine journal. “In the future, we hope it will improve early detection and potentially make cancer treatment more successful by highlighting high-risk patients and fast-tracking them to earlier intervention,” said Dr Benjamin Hunter, a clinical oncology registrar at the Royal Marsden and a clinical research fellow at Imperial.     Community hospitals are an important part of local health and care systems, yet there has been very little shared on their role and contribution during the pandemic. This project from the Community Hospitals Association sought to redress this and highlight the role of these local hospitals. This two-year project enabled staff to reflect on their experiences and innovations in their community hospitals during the pandemic in a systematic way that facilitated wider sharing and learning. It captures the experiences of staff working in UK community hospitals during the COVID-19 pandemic, with a focus on positive impact changes.

    A significant amount of professional time is wasted during a medical ward round retrieving patient notes from the ward trolley. If the efficiency of this non-clinical, non-functional interaction could be improved it would save time, maintain continuity and have financial implications. One identified constraint was the structure of the traditional ward trolley; a stationary filing tray with vertical sleeves. During ward round, time is spent returning and retrieving each patients notes from outside the patient bay and additional time may be wasted if the notes are misplaced or in use elsewhere. To resolve this, the ‘Vista 90’ trolley with horizontal, transparent trays, is portable and has an ergonomic writing surface was selected as a potential second generation replacement. An assessment of the impact of the Vista 90 trolley over the traditional trolley in the clinical setting was carried out on Erringham (medical) Ward, Worthing Hospital, West Sussex Hospital Trust, UK. This was by way of qualitative analysis performed by semi-structured interview of 12 doctors and other healthcare professionals who regularly interacted with the Vista 90 and traditional trolley in December 2012. The audit found that those interviewed preferred using the Vista 90 trolley over its predecessor as it improved the efficiency of the ward round and subsequent clinical work. It’s mobility allowed it to be easily transported with the ward round, reducing disruption during a consultation and between consecutives ones. The ergonomic writing surface was noted to improve legibility of documentation due to greater comfort and if placed appropriately, did not interfere with the doctor-patient interaction. The financial savings of this greater efficiency was found to be of significance and justify the cost of the Vista 90 within two weeks.

    Digital transformation across adult social care is occurring rapidly, however, uptake is not uniform, and the care sector is yet to fully harness digital tools to transform care delivery. With unprecedented service pressure and demand across health and care services, using digital tools in care settings has the potential to relieve some pressure by increasing efficiency and better supporting the workforce. This report by the think tank Public Policy Projects brings together the thoughts and ideas of many Adult Social Care experts regarding the future of the care sector, and the opportunities which digital advancements can bring.     This paper in the journal RSC Advances aimed to track changes in chemical bonding taking place in PP meshes on the nanoscale via mechano–chemical processes. The authors used the novel and advanced spectroscopic characterisation technique secondary electron hyperspectral imaging (SEHI) to build high resolution chemical maps. Polypropylene (PP) surgical mesh is associated with serious clinical complications when used in the pelvic floor for repair of stress urinary incontinence or support of pelvic organ prolapse. While manufacturers claim that the material is inert and non-degradable, there is a growing body of evidence that asserts PP fibres are subject to oxidative damage. Material surgically removed from patients suffering with clinical complications has shown some evidence of fibre cracking and oxidation. It has been proposed that a pathological cellular response to the surgical mesh contributes to medical complications, but the mechanisms that trigger the specific host response against the material are not well understood.  The study presented key insights into the mechano–chemistry reaction of PP which can cause polymer oxidation, changes in molecular structure, crack/craze formation and the release of etched oxidised insoluble particles. SEHI, provided a new route to link the effect of localised stresses to reactions of mechano–chemistry within PP. The method of mechanical distension testing during hydrogen peroxide exposure followed SEHI image analysis could form the basis of an “early warning” system which has the ability to identify materials which are not appropriate for use as medical implants.

    This case study published by the Healthcare Quality Improvement Partnership (HQIP) highlights the Epilepsy12 Audit’s approach to working with children and young people to improve paediatric epilepsy care. Epilepsy12 Youth Advocates are epilepsy experienced or interested children, young people, families and an epilepsy specialist nurse. They volunteer together to shape Epilepsy12 and to lead improvement activities with families and epilepsy services. The audit won the Richard Driscoll Memorial Award (RDMA) 2022. The RDMA asks HQIP commissioned programmes to describe how patients and carers influence the production of the patient-focused outputs of the programme.

    Cancer Research UK, in partnership with London-based tech company Stitch, are piloting an app for patients to use whilst participating in a clinical trial. The Trialmap app, which was co-created with patients, is being piloted on a clinical trial run by Cancer Research UK’s Centre for Drug Development. The aim of the app is to ensure patients feel valued for their participation, and to improve patient experience during clinical trials. This article looks at how the app: allows patients to easily view information about the trial gives reminders about appointments and what patients might need to do to prepare for them gives patients the opportunity to provide real-time feedback regarding their time on the trial.

    Tracey Herlihey, head of patient safety incident response policy, in the NHS England national patient safety team, is joined by Vicky Ainsworth, a communications lead at Manchester University NHS Foundation Trust and Stuart Kaill, from Health Innovation Manchester, to discuss ways of communicating about large scale change projects in NHS organisations. The podcast explores Vicky’s experience of leading on communications for a large scale change project in Manchester, with a specific focus on sharing advice and suggestions relating to communicating the changes related to the Patient Safety Incident Response Framework (PSIRF). It includes expert tips on how to communicate large scale change to different audiences as well as within both large and small organisations.

    Innovation and technology have played a vital role in our health system, not least during the pandemic. Whilst we have seen a variety of positive changes and reforms, such as the MedTech Funding Mandate and new data plans, the new administration still has a lot to address. Not forgetting the positive lessons we learnt from the pandemic is key – we saw that a more rapid uptake is possible and barriers to adoption can be overcome. Addressing these barriers to adoption is still essential if the UK wants to be the global hub for technology and innovation – adopting technological driven innovative solutions will be more important than ever for the NHS in order to continue improving patient outcomes whilst maintaining economic discipline.
