    In this study, Mansab et al. examine the COVID-19 community triage pathways used by Singapore, Japan, USA and UK, specifically comparing the safety and efficacy of national online ‘symptom checkers’ used within the triage pathway.Their results suggest that whilst ‘symptom checkers’ may be of use to the healthcare COVID-19 response, there is the potential for such patient-led assessment tools to worsen outcomes by delaying appropriate clinical assessment. The key features of the well-performing symptom checkers are discussed in the paper.

    The Digital Clinical Safety Strategy is a joint publication between NHSX, NHS Digital and NHS England and NHS Improvement. It is an addendum to the NHS Patient Safety Strategy, outlining the case for improved digital clinical safety across health and social care. The aim of the strategy is twofold: To improve the safety of digital technologies in health and care, now and in the future. To identify, and promote the use of, digital technologies as solutions to patient safety challenges. In other words, digital clinical safety is about making sure the technologies used in health and care are safe, and then using those technologies to improve patient safety.

    Researchers have developed an artificial intelligence (AI) tool for rapidly detecting COVID-19 in people arriving at a hospital’s emergency department. The tool can accurately rule out infection within an hour of a patient arriving at hospital, significantly faster than the PCR (polymerase chain reaction) test that has a turnaround time of typically 24 hours.

To support the Health Foundation’s COVID-19 impact inquiry, the Good Things Foundation, the Health Foundation and the King’s Fund came together earlier this year, to focus on tackling digital exclusion and health inequalities. The resulting report from the workshop and meeting provides an overview of digital exclusion and who is affected by it, as well as the impact of the pandemic on this, and some of the policy responses so far.

    The use of digital health services has risen over the course of the COVID-19 pandemic. The digital divide and the resulting impact on people’s experiences of the pandemic have disproportionately affected certain groups of society.  Age UK analysis suggested that only 24% of those aged 75+ were using the internet more during the pandemic, and 9% were using it less. And although the population has become better connected since the start of the pandemic, still 6% of homes (around 1.5 million households) in the UK lack home internet access. People in the poorest households are four times more likely to not use the internet at home than those in the wealthiest households. Disability, impairment, and health conditions also correlate with lower levels of digital access and use.     Evidence regarding the value of virtual care tends to narrowly focus on short-term measures of financial value. There remains little literature regarding the long-term effects of virtual care, such as improvements in access to care, clinical outcomes, the impact on the patient and clinician experience, the potential for operational efficiencies, or the impact on health equity.     Artificial intelligence tools and deep learning models are a powerful tool in cancer treatment. They can be used to analyse digital images of tumour biopsy samples, helping physicians quickly classify the type of cancer, predict prognosis and guide a course of treatment for the patient. However, unless these algorithms are properly calibrated, they can sometimes make inaccurate or biased predictions, as Howard et al. demonstrate in this study.

    This Rapid Evidence Scan from Moore et al. examined the effectiveness of virtual hospital models of care. While no reviews evaluated a complete model, tele-healthcare only and tele-healthcare with remote telemonitoring interventions demonstrated similar or significantly better clinical or health system outcomes including reduced hospitalisations, readmissions, emergency department visits and length of stay, compared to usual care, including those delivered without home visits or face-to-face care. The use of the Internet showed mixed but promising results. The strongest evidence was for cardiac failure, coronary heart disease, diabetes and stroke rehabilitation. Nurses played a central role in home visiting, providing telephone support and education. However, the studies were heterogenous and the results should be interpreted with caution.

    In healthcare, digital solutions have been adopted with zeal, but there is paucity of evidence for benefits and harms of these solutions. The impact, immediate or long term, of digital applications on healthcare has not been assessed. With the overwhelming numbers and types of digital solutions, it is becoming increasingly important to develop evidence-based insights for the integration of these solutions in routine medical care. Digitalisation can certainly empower and enable patients and physicians to achieve health objectives. The World Health Organization has released guidance for digital health after a critical review of available evidence for the benefits, harms, acceptability, feasibility, resource use and equity considerations of digital health interventions. This guidance can potentially inspire and impact future research endeavours for digital applications. Tarveen Jandoo reviews the guidance in context of the current research situation and insights are shared for researchers engaged in the design and assessment of digital interventions.

    This article in BMC Health Services Research looks at a range of macro, meso and micro factors influencing eHealth innovation in the English NHS. eHealth is a broad term which encompasses e-health, m-health, telemedicine and telecare, public health surveillance, personalised medicine/patient engagement, health and medical platforms, self-tracking, medical imaging, healthcare information systems, mobile connectivity, social networking, sensors and wearables, gamification, electronic health records, big data, health information technology, health analytics, digitised health systems, robotics and active assistive living. The study found that the fragmentation of the NHS is the most significant factor limiting the adoption of eHealth innovations, arguing that national policy has intensified the digital divide. It states that the NHS Long Term Plan places great emphasis on the role of digital transformation in aiding communication and enabling people to access care quickly and easily, highlighting significant implications for effectiveness, efficiency and equity.

    In this 'letter', Dr Soojin Jun, as a healthcare professional and a patient advocate, gives her three recommendations to guard "patient safety" in the digital health era. Your end-users are ultimately patients, no matter who uses your product.  Healthcare is not binary, and your digital solutions shouldn’t be either.     The COVID-19 pandemic continues to be a source of stress and have important mental health implications for all persons but may have unique implications for men. In addition to the risk of contracting and dying from COVID-19, the rising COVID-19 death toll, ongoing economic uncertainty, loneliness from social distancing, and other changes to our lifestyles make up the perfect recipe for a decline in mental health. In June 2020, men reported slightly lower rates of anxiety than women, but had higher rates of depressive symptoms and suicidal ideation. As of September 2020, men sought mental health care at a higher rate than women for family and relationships, with year-over-year visits up 5.5 times and total virtual mental health care visits monthly growth in 2020 was up 79% since January. Because men are not a homogeneous group, it is important to implement strategies for groups of men that may have particularly unique needs. In this paper, Ellison et al. discuss considerations for intervening in men’s mental health during and in response to the COVID-19 pandemic, including current technology-based cyberpsychology options.
