Summary
Patient Safety Learning and the Safer Healthcare and Biosafety Network (SHBN) are undertaking a project, working with patient safety experts and frontline staff, to produce a manual to support staff after a serious safety incident. As part of this work, we are asking healthcare staff to complete a short survey relating to experiences of a serious safety incident.

Patient safety and staff safety are intrinsically linked - you cannot effectively have one without the other. One aspect of staff safety that is particularly closely intertwined with patient safety is support for staff following patient safety incidents. Having the right support in place can help foster an environment of openness; one of talking about patient safety and, consequently, supporting a culture of learning from patient safety incidents.
We know that serious incidents can have a significant impact on staff who were involved or who may have witnessed the incident. There is strong evidence that staff are not always supported or treated fairly in incident investigation, that culpability mechanisms often take precedence over learning and that staff are not always treated fairly or supported.
Patient Safety Learning and SHBN are collaborating with patient safety experts and frontline staff to produce a manual to support staff, and provide good practice and ‘how to’ tools to improve staff wellbeing following serious safety incidents.
To inform this work we would like to hear from healthcare staff about their experiences of serious safety incidents, the level of support they received and what support they may find helpful.
