Summary
Cheshire and Merseyside Health and Care Partnership has published this consensus document on the interface between primary and secondary care. It aims to ensure healthcare providers ensure access to the right care to give patients the best outcomes. It contains a set of clinically-led principles to ensure pathways have a common structure of good quality, patient-centred communication.
It includes a number of guiding principles that encourage staff to ensure:
- the patient is at the centre of decision making
- actions taken are completed in a timely way
- actions are undertaken by the most appropriate individual or team
- decisions and actions are understood by all.
