	We have been attempting to complete PSIRF/PSII's templates, we have kind of been dropped in at the deep end and haven't really had much guidance it how to do this so we are just trying different ways, the templates we have been using prior to PSII's are the SI and CI templates. We have contacted different Trust who were early PSIRF adopters, these were helpful, but we are still no where near ready to roll this out and are not sure what is expected of us, we are completing SWARM huddles and AAR, we are shadowing meetings where incidents have been found to need further investigations. We are just really at a loss at the minute and would love to hear from Trusts/groups that are in 'full swing' with PSIRF/PSIIs to find out how you are getting on with them, how you started out, any training guides of what is expected etc. Any help with PSII's would be most appreciated, any process maps/SOP's/ other PSII templates. I guess we just want to know how you started it out and then rolled it out within your Trust, how did you get 'buy in' from staff etc, how do you capture data for this i.e. themes around incidents.

	Good morning colleagues. I am the National manager for Patient Safety &amp; Quality, MOHMS, Fiji.

	I am searching for RCA Training Guidelines (manuals) that I can use as a reference to review and develop our current training manual on conducting RCAs in our health care settings. I have not been successful so far in finding one online.
