DESCRIPTION:\n	This national conference looks at the practicalities of S
	erious Incident Investigation and Learning from Deaths in Mental Health Se
	rvices. The event will look at the development and implementation of the N
	ew Patient Safety Incident Response Framework (previously known as the Ser
	ious Incident Framework). NHS Improvement is working with these early adop
	ters to test implementation\, and analysis of this will inform the final v
	ersion with the revised framework due in Spring 2022. Local systems and or
	ganisations outside of the early adopter areas are free to use the already
	 published version of the PSIRF to start to plan and prepare for PSIRF’s
	 full introduction.\n\n\n\n	The conference will also update delegates on b
	est current practice in serious incident investigation and learning\, incl
	uding mortality governance and learning from deaths. There will be an exte
	nded focus on ensuring serious investigation findings lead to change and i
	mprovement\, and updates from PSIRF early adopter sites in mental health. 
	The conference will also examine how the new framework will fit with the R
	oyal College of Psychiatrists Care Review Tool for mortality review.\n\n\n
	\n	This conference will enable you to:\n\n\n\n	\n		Network with colleagues
	 who are working to improve the investigation of serious incidents and dea
	ths in mental health services.\n	\n	\n		Ensure your approach to Serious In
	cident Investigation is in line with the Patient Safety Incident Response 
	Framework (PSIRF).\n	\n	\n		Learn from outstanding practice in implementin
	g the Royal College of Psychiatrists Mortality Care Review Tool.\n	\n	\n		
	Reflect on the lived experience of a bereaved relative.\n	\n	\n		Improve t
	he way you involve and engage families and carers in the investigation pro
	cess.\n	\n	\n		Develop your skills in incident investigation and mortality
	 review.\n	\n	\n		Understand how you can improve serious incident investig
	ation and learn from Mental Health early adopters of the New Patient Safet
	y Incident Response Framework. \n	\n	\n		Identify key strategies for unde
	rtaking a self assessment\, and continuous review of deaths and investigat
	ion practice in your organisation.\n	\n	\n		Understand how human factors c
	an help improve learning from serious incident investigation.\n	\n	\n		Ens
	ure you are up to date with the role of the coroner.\n	\n	\n		Understand h
	ow you can better support staff when a serious incident occurs. \n	\n	\n

	s 5 Hrs training for CPD subject to peer group approval for revalidation
