	for long-term physical and mental health conditions and specialist palliat
	ive care at the end of their lives.\n\n\n\n	Better partnership working bet
	ween the housing\, health and social care sectors and care homes could enh
	ance the wellbeing of residents and reduce the number of avoidable hospita
	l admissions from care homes. It could also enable older people to have mo
