	ed quality improvement in individual GP practices across Primary Care Netw
	orks (PCNs). It will support primary care in understanding how many patien
	ts with CVD and/or the six main high-risk conditions are potentially undia
	gnosed\, or under or over treated. These include atrial fibrillation\, hig
	h blood pressure\, high cholesterol\, diabetes\, non-diabetic hyperglycaem
	ia and chronic kidney disease. The audit will provide data to highlight ga
