	CIEHF) recently published a White Paper on ‘Learning from Adverse Even
	ts’. Based on nine principles\, the paper presents the basis of what th
	e Institute considers Human Factors good practice in investigating and lea
	rning from adverse events. The emphasis on learning\, rather than liabilit
	y\, has a number of important implications\, not least in the nature of th
	e evidence needed to support decisions and actions. This Q Community webin
	ar will explain the background to the paper and introduce each of the nine
