DESCRIPTION:\n	RCA continues to be used around the world by healthcare t
eams to better understand and learn from their patient safety incidents. W
hile it is no longer an approved learning response under the NHS Patient S
afety Incident Response Framework (PSIRF)\, many care and non NHS organisa
tions continue to use RCA as a structured and effective approach for under
standing incidents and improving safety.\n\n\n\n	This one-day intensive an
d interactive masterclass will provide insights into how RCA is being used
effectively by healthcare and non-healthcare organisations across the wor
ld to enhance safety. RCA relies on the creation of a skilled team of inve
stigators\, working together to look beyond surface-level symptoms. By fol
lowing a clear seven-step process and by embracing a range of established
RCA techniques\, healthcare teams can collaborate to make their patients s
afe.\n\n\n\n	The course is facilitated by Tracy Ruthven and Stephen Ashmor
e who have significant experience of undertaking patient safety reviews in
healthcare. They were commissioned to write a national RCA guide by the H
ealthcare Quality Improvement Partnership.\n\n\n\n	This course supports CP
D professional development and acts as revalidation evidence. This course
provides 5 Hrs training for CPD subject to peer group approval for revalid
	ation purposes.\n\n\n\n	Register\n\n
