DESCRIPTION:\n	Cardiovascular disease (conditions affecting the heart or
	 blood vessels) accounts for around 1 in 4 deaths in the UK each year.\n\n
	\n\n	A person’s risk of cardiovascular disease depends on their blood pr
	essure\, weight\, cholesterol\, smoking status\, family history\, and othe
	r factors. Identifying an individual’s risk allows preventative measures
	\, such as changes in lifestyle or taking medicines\, to reduce risk and i
	mprove health outcomes.\n\n\n\n	An assessment of cardiovascular risk is pa
	rt of routine care\, during the NHS Health Check\, for example\, but more 
	can be done to optimise the identification of risk for the future. Join th
