DESCRIPTION:\n	This National Conference focuses on improving the investi
	gation and learning from deaths and will update delegates on the death cer
	tification reforms which came into force in September 2024. There will be 
	an extended focus on engaging and involving patients\, families and staff 
	following a death\, and on learning from deaths including an update from a
	 coroner.\n\n\n\n	The conference will discuss learning from deaths which i
	s now being extended to all non-coronial deaths wherever they occur and wi
	ll provide a practical guide to learning from deaths and improving practic
	e in your service. The conference will also update delegates on the Nation
	al Patient Safety Incident Response Framework (PSIRF) and the implications
	 for patient safety incident investigation and learning from deaths.\n\n\n
