	ng issue for over two decades\, with the first Patient Safety Alert (PSA
	) issued back in 2005\, drawing attention to the dangers of misplaced NGT
	s.\n\n	\n\n	Despite subsequent alerts\, reports\, and detailed studies —
	 including an investigation by the Health Service Safety Investigation Bo
	dy (HSSIB) in 2020 — little progress has been made\, and misplaced NGTs
