DESCRIPTION:\n	This conference will provide a focus on the pivotal issue
	s concerning the future strategies of both the NHS Long Term Plan (LTP) an
	d and integrated care systems (ICS) and will incorporate key elements of t
	he 'Major Conditions Strategy' that is currently being developed.\n\n\n\n	
	This major conditions strategy theme within the conference begins with one
	 question: how should our approach to health and care delivery evolve to i
	mprove outcomes and better meet the needs of our population\, which is bec
	oming older and living with multimorbidity? With varying groups of conditi
	ons being explored in our plenaries and workshop sessions: cancers\, cardi
	ovascular disease (CVD) (including stroke and diabetes)\, mental ill healt
	h\, and chronic respiratory disease.\n\n\n\n	There are signs of positivity
	 with exciting new technologies and sources of data that enable staff to b
	etter see and treat disease at an earlier stage. Our conference and its de
	dicated plenaries\, evidence-based case studies and educational workshops 
	will explore how to supercharge this agenda. Amongst our core themes will 
	be the exploration of the potential of AI and technology to deliver digita
	l transformations\, and position the UK as a world leader in virtual healt
	h and social care. As such topics such as Virtual Wards\, Remote Patient a
	nd Whole Patient Management systems will be at the forefront of certain pl
	enaries.\n\n	 \n\n	Also underpinning the conference's focus are the stron
	g desires to tackle health disparities and promoting inclusion in health\,
	 as articulated through the NHS Core20PLUS5 framework\, and detailed by se
