DESCRIPTION:\n	This one day masterclass is part of a series of masterc
	lasses focusing on how to use Human Factors in your workplace and is align
	ed with the new Patient Safety Syllabus and subsequent Patient Safety Inci
	dent Response Framework (PSIRF).\n\n\n\n	The new Patient Safety Strategy a
	dvises that organisations must adopt a new and broader approach to stimula
	te learning from patient safety incidents. This course is designed to assi
	st healthcare professionals involved in this important work.\n\n\n\n	The m
	ain purpose is to provide learners with a full understanding of the variou
	s approaches that can now be used to conduct patient safety incident inves
	tigation (PSIIs). Traditionally\, root cause analysis has been used as a b
	lanket approach to diagnosing why patient safety have been compromised\, b
	ut healthcare teams are henceforth being encouraged to adopt a wider range
	 of methods that will both save time and facilitate enhanced learning. The
	 focus is now on appropriate proportionality in response to incidents that
	 occur in their organisation.\n\n\n\n	Key learning objectives:\n\n\n\n	\n	
		Understand the new patient safety landscape\n	\n	\n		Understand the need 
	for proportionality of investigation\n	\n	\n		Learn how to use a range of 
	techniques for conducting PSIIs\n	\n	\n		Understand how to write an impact

	t safety investigations compares to the agreed national standards\n	\n	\n

	racy Ruthven and Stephen Ashmore who have significant experience of und
	ertaking patient safety reviews in healthcare. They were commissioned to w
	rite a national RCA guide by the Healthcare Quality Improvement Partnershi
	p. They have also authored articles on significant event analysis and clin
	ical audit/quality improvement\, all techniques seen as increasingly relev
