DESCRIPTION:\n	Launch event for PHSO’s new report\, A place of safety
? Learning from avoidable harm in the NHS.\n\n\n\n	Date: Thursday 29 June

m to discuss their findings and recommendations about improving patient sa
fety. Chief Executive Amanda Amroliwala and the Ombudsman Rob Behrens CBE
