DESCRIPTION:\n	This two day intensive masterclass will provide Root Caus
	e Analysis Training in line with the July 2019 Patient Safety Strategy.\n\
	n\n\n	This intensive two day masterclass will provide Root Cause Analysis 
	training in line with the 2019 Patient Safety Strategy and subsequent guid
	ance. The course will offer a practical guide to conducting RCA with a foc
	us on systems-based patient safety investigation as proposed within the la
	test guidance released by NHS England and NHS Improvement. The course prov
	ides insights into how RCA is evolving and gives detailed information on w
	hat standards RCA investigations are expected to reach following the detai
	led recent reviews of patient safety work across the NHS and healthcare. 
	 \n\n\n\n	The new National Patient Safety Incident Response Framework (PS
	IRF) published in 2020 highlights important changes to the way safety inci
	dents will be investigated\, which is reflected in this course. Key new co
	ntent includes:\n\n\n\n	\n		Promotion of RCA as a tool for learning &amp\;
	 improvement.\n	\n	\n		Emphasis on increased use of listening &amp\; inter
	view (staff and patients) to gain a better understanding of what has happe
	ned.\n	\n	\n		The importance of Safety II and focusing on system strengths
	\, plus linking RCAs to QI &amp\; clinical audit.\n	\n	\n		More emphasis o
	n human factors.\n	\n	\n		Brief information on approaches that may be more
	 appropriate to RCA (e.g. significant event analysis\, after-action review
